The Subject Of The Order Is Service Of Training And Related Forms Of Support As Part Of The Project Coordinated Care In Primary Health Care 2. The Subject Of The Order Is Carried Out In Connection With The Project: Coordinated Care In Primary Health Care As Part Of The European Funds For Social Development Project 2021-2027 Co-Financed By The European Social Fund Plus Under The Contract: Fers.04.15-Ip. P.07-0003/23-00/1374/2024/217). The Subject Of The Order Includes: 1) Analysis And Evaluation Of The Implementation Of Care Coordination Solutions In Primary Care; 2) Hybrid And Online Training On Coordinated Care, Diagnosis, Treatment And Prevention; After The Completion Of The Training. 3. The Ordering Party Additionally Envisages The Use Of The Option Right, Consisting In Conducting Up To 30 Additional Training Sessions For Training Participants, On The Terms Identical To The Description Of The Subject Of The Order For The Basic Order. 1) The Option Will Be Activated Provided That The Value Of The Basic Order Together With The Option Is Within The Estimated Value Intended For The Execution Of The Order By The Ordering Party. 2) In Order To Exercise The Option Right, The Ordering Party Shall Submit To The Contractor A Written Declaration On The Exercise Of The Option Right, No Later Than Within One Month Before The Expiry Date Of The Agreement. 4. The Description Of The Subject Of The Order Is Included In Annex No. 1 To The Specification. 5. The Ordering Party Does Not Envisage Awarding Orders Pursuant To Art. 305 In Connection With Article 214(1)(7) Of The Public Procurement Law.
