1) The Order Includes The Supply Of Equipment For The Care And Housing Center In Siedliszowice. The Description Of The Subject Of The Order For Part 1 - Delivery Of Rehabilitation And Medical Equipment Is Specified In: • Assortment And Price Form - Annex No. 4.1 To The Swz • Draft Provisions Of The Contract - Annex No. 5 To The Swz.3) The Equipment Must Be New, Unused, And Cannot Be The Subject Of Enforcement Or Administrative Proceedings Or Be The Subject Of Security.4) Together With The Delivered Assortment, The Contractor Will Provide The Approvals, Certificates, Operating Instructions, Assembly Instructions And Warranty Cards Required For The Given Assortment. 6) The Contractor Is Obliged To Deliver, Unload, Bring And Unpack The Assortment Included In The Subject Of The Order To The Care And Housing Center In Siedliszowice, Located At: Siedliszowice 181, 33-250 Otfinów. With Respect To The Assortment Which, Due To Its Nature Or Purpose, Requires Assembly, Setting, Connection Or Commissioning, The Contractor Is Obliged To Perform These Activities And Hand Them Over To The Ordering Party In A Ready-To-Use Condition.7) The Ordering Party Expects The Contractor To Provide The Price For The Execution Of The Subject Of The Order As A Lump Sum Price, Which Includes The Execution Of All Elements Of The Order In Accordance With The Description Contained In The Annexes Detailing The Given Part Of The Subject Of The Order For Which The Contractor Submits An Offer.8) The Contractor Is Responsible For Carefully Reading The Order Documents In Order To Estimate, On Your Own Responsibility, The Costs And Risks Of Order Execution.9) All Requirements Specified In The Documents Indicated In Point 2) Above Are Minimum Requirements And Their Fulfillment Is Obligatory. Failure To Meet The Above-Mentioned Minimum Requirements Will Result In Rejection Of The Offer As Inconsistent With The Terms Of The Contract Pursuant To Art. 226 Section 1 Point 5 Of The Public Procurement Law.
