Part I. Educational Meetings, Workshops And Training In The Field Of Prevention Of Addictions And Behavioral Disorders For Students Of Grades 1-3, 4-6 And 7-8 Of Primary Schools And For Students Of Secondary Schools Run By The Nowogard Commune 1. The Subject Of The Order Will Be Implemented In Accordance With The Provisions Of The Specification Of The Terms Of Order Together With Annexes, Using Materials And Document Templates Provided By The Ordering Party. 2. The Contractor Is Obliged To Execute The Subject Of The Order On Time And Efficiently And To Cooperate With The Ordering Party On An Ongoing Basis At Every Stage Of Providing Services. 3. The Contractor Is Obliged To Carry Out The Task In Accordance With Applicable Regulations And Occupational Health And Safety (Osh) Principles. 4. The Contractor Will Ensure Proper Protection Of Personal Data Of Participants/Support Forms In Accordance With Applicable Law And The Ordering Partys Requirements. 5. The Participants Of The Forms Of Support That Are The Subject Of The Order Are Children And Young People At School: Students Of Primary Schools (7-14 Years Old) And Secondary Schools (15-19 Years Old). 6. Services Will Be Provided On Business Days (Monday To Friday, Excluding Public Holidays). The Possible Time Frame For Classes Is From 7:30 A.M. To 3:00 P.M., And The Actual Dates And Hours Of Individual Forms Of Support Will Be Specified In Detailed Schedules. 7. A Detailed Schedule Of Classes Will Be Established After Signing The Contract, In Consultation With The Contractor And Taking Into Account The Availability Of Project Participants. 8. Any Changes In The Schedule Must Be Notified To The Ordering Party At Least One Day Before The Planned Meeting Date. 9. The Service Will Be Provided In Rooms Provided Free Of Charge To The Contractor By The Ordering Party - List Of Schools Above. The Therapists Psychologists Office Will Be Located In The Building Of The Nowogard Commune. The Contractor Is Obliged To Use The Provided Premises Solely For The Purpose Of Carrying Out The Task And Only During The Hours Specified In The Approved Schedule. 10. The Contractor Is Obliged To Inform Each Participant Before Starting The Task That The Service Is Co-Financed By The European Union Under The European Social Fund+. 11. The Contractor Is Obliged To Keep Documentation (In Particular Attendance Lists And Schedules) Only On The Template Forms Provided By The Ordering Party. 12. The Contractor Is Obliged To Document The Presence Of Participants By Collecting Their Handwritten Signatures On Attendance Lists. 13. The Contractor Is Obliged To Enable Monitoring Visits To Be Carried Out At The Place Where The Service Is Provided. Monitoring May Be Carried Out By Authorized Employees Of The Municipal Office In Nowogard In Order To Verify The Quality And Compliance Of The Support Provided With The Schedule. 14. Obtaining Two Negative Assessments From The Monitoring Activities Carried Out Constitutes The Basis For Termin
