Employed Under An Employment Contract, Persons Employed Under A Management Contract, Job Candidates (Hereinafter Referred To As The Employee Or Employees) Only For Initial Occupational Medicine Examinations, One Person Indicated By The Employee As A Person Being The Wife/Husband Or Partner (The Total Cost Of Additional Medical Care Without Rehabilitation Is Borne By The Employee) An Unlimited Number Of The Employees Children (The Total Cost Of Additional Medical Care Without Rehabilitation Is Borne By The Employee), On The Basis Of The Law That Regulates The Issues Of Health Prevention For Employees, In Particular: 1) Act Of June 26, 1974, Labor Code (Journal Of Laws Of 2026, Item 25), 2) Regulation Of The Minister Of Health And Social Welfare Of May 30, 1996 On Medical Examinations Of Employees, The Scope Of Preventive Health Care For Employees And Medical Certificates Issued For The Purposes Of The Labor Code (Consolidated Text, Journal Of Laws Of 2026, Item 25) And Providing Medical Services In The Field Of Vaccinations. The Planned (Estimated) Number Of People Who Will Be Covered By The Provision Of Occupational Medicine Services, Additional Medical Care And Rehabilitation During The Duration Of The Contract (24 Months) Is Specified As Follows: Occupational Medicine - 1,500 People; Additional Medical Care - 1,050 People; Rehabilitation As Part Of Additional Medical Care (Only For Employees) - 1,050 People; Additional Medical Care For Wife/Husband, Partner - 160 People; Additional Medical Care For Children Up To 18 Years Old - 155 People; Additional Medical Care For Children Aged 18-26 - 155 People.
