Medical Consumables – Consumable Medical Material. The Subject Of The Performance Of This Public Contract Is The Conclusion Of The Framework Purchase Agreement (Hereinafter Also Referred To As The Agreement) For The Continuous Supply Of Medical Consumables (Hereinafter Referred To As Goods), Which Is Intended For The Provision Of Pre-Hospital Emergency Care. The Contract With One Selected Supplier Will Be Concluded For A Period Of 2 Years. The Amount Of Medical Consumables Will Be Ordered By The Client According To Current Needs. Performance Will Be Based On Written Requests (Orders) To Provide Partial Performance. More Detailed Information On The Subject Of Performance Is Given In The Document Specification Of The Subject Of Performance (Goods) And Price List (Appendix No. 4 Zd). The Client Requests Supplies Of Consumable Medical Supplies In The Range Of Items Listed In Appendix No. 4 Zd (A Positive Sheet Where Individual Items Are Listed Alphabetically). The Supplier Must Submit An Offer For All Items That The Contracting Authority Needs To Ensure Pre-Hospital Emergency Care - I.E. For The Entire Required Assortment Of Medical Consumables, So That Its Offer Can Be Evaluated And Compared With Other Suppliers Offers. Min. The Requirements For The Specification Of The Goods Are Listed In Appendix No. 4 Zd. If The Performance Specification Contains References To Individual Trade Names, Special Designations Of Products, Useful Types Or Commercial Materials That Are Characteristic Of A Certain Supplier, The Contracting Authority Allows The Use Of Other, Technically And Qualitatively Similar Or Clearly Better Products (Medical Consumables), Which, However, Must Fully Meet The Minimum Requirements Of The Contracting Authority And Meet The Conditions Of Medical Purpose, Diagnostic And Therapeutic Use For The Given Product. In Cases Where A Specific Name Is Given In Appendix No. 4 And Appendix No. 5 Of The Zd (E.G. Vidacare Drill), This Is The Clients Existing Equipment, Therefore The Offered Goods Must Be Fully Compatible With The Clients Existing Equipment. The Specification Of The Goods For Which The Client Requires The Submission Of Samples As Part Of The Offer Is Specified In The Specification Of The Submitted Samples (Appendix No. 5 Zd). The Shelf Life Of The Goods Must Be At Least 1 Year. The Supplier Will Be Responsible For The Fact That The Delivered Goods Are Suitable For Use In Accordance With Their Intended Use And Meet All The Requirements Of The Relevant Legal Regulations, In Particular Act No. 375/2022 Coll. On Medical Devices And In Vitro Diagnostic Medical Devices, As Amended (Relevant For Goods That Have The Status Of A Medical Device). The Supplier Will Be Responsible For The Fact That The Delivered Goods Will Be Transported In Accordance With The Applicable Current Regulations. As Part Of The Subject Of Performance, The Contracting Authority Requires The Delivery Of A Declaration Of Conformity For The Affected Supplies Of Consumable Medical Equipment (The Declaration Will Be Part Of The Tax Document/Invoice Or Delivery Note).
