Tenders Are Invited For Social Services Social Taxi

1. Social Services Social Taxi Will Be Provided To Residents Of The Biesiekierz Commune Qualified To Participate In The Cus Plus Project (Hereinafter Referred To As Project Participants, Beneficiaries, Recipients).2. The Subject Of The Order Is The Provision Of The Social Taxi Transport Service, Consisting In Transporting Project Participants From Their Place Of Residence To Their Destination And From The Destination Back To Their Place Of Residence. The Service Will Be Provided In The Biesiekierz Commune And - In Justified Cases - In Its Direct Functional Area. The Service Includes Comprehensive Transport (To And Back): A) From The Place Of Residence To The Medical/Rehabilitation Facility, B) To The Office, C) To A Designated Place In Order To Fulfill A Spiritual Need, D) For Shopping (Food, Medical Or To Provide Another Service, E.G. Handyman), And Providing Assistance (Driver) On The Way To/From The Vehicle, Apartment, Destination, Stopover/Waiting.3. The Service Is Provided In A Unit Of Measurement - A Service As Part Of A Service Basket. One Service Is Understood As Comprehensive Transport Of One Project Participant, Including: A) Travel From The Place Of Residence To The Destination, B) Waiting (Stopping), If Required - Up To 1 Hour, C) Return Journey From The Destination To The Place Of Residence.4. The Social Taxi Service Basket Includes: A) Comprehensive Transport Of A Project Participant Up To 8 Km One Way, A Maximum Of 16 Km In Both Directions, B) Comprehensive Transport Of A Project Participant Up To 10 Km One Way, A Maximum Of 20 Km In Both Directions, C) Comprehensive Transport Of A Project Participant Up To 15 Km One Way, A Maximum Of 30 Km Both Ways, D) Comprehensive Transport Of A Project Participant Up To 20 Km One Way, Maximum 40 Km Round Trip.5. The Total Number Of Services Is A Maximum Of 300, A Minimum Of 200. The Scope Of Services For One Participant Will Depend On The Needs, But No More Than 3 Trips A Month Or, In The Case Of Rehabilitation, Up To 10 Trips A Month.6. A Detailed Description Of The Subject Of The Order Along With The Documents Required For The Execution Of The Order Can Be Found In Annex No. 5 To The Swz.
