Task No. 2 - Nursing Support Service.2.2. Description Of Task No. 2. Nursing Support Service. Detailed Description Of The Subject Of The Order: Provision Of Care Services At The Place Of Residence By A Nurse For People Who Need Support In Their Daily Functioning Due To Their Health Condition, Age Or Disability, Who Require Care Or Support Due To The Inability To Independently Perform At Least One Of The Basic Everyday Activities, As Well As Providing Advisory Support By A Nurse In The Form Of Specialist Counseling For Caregivers Of Actual People Supported By Care Services At The Place Of Residence, Who Have Difficulties Or Showing The Need For Support In Solving Life Problems, The Aim Of Which Will Be To Identify The Patients Health Conditions And Needs, Identify Care Problems, Health Education And Health Promotion In Order To Prevent Diseases And Deterioration Of Health. The Scope Of Services Provided Includes: 1) Visits To The Patients Place Of Residence And Meetings With A Caregiver Aimed At Educational Activities Regarding The Proper Functioning Of A Specific Dependent Person In Home Conditions, In Particular: Education In The Prevention Of Infections And Contagions, Education In The Field Nutrition Resulting From The Health Condition, Education In The Prevention Of Water And Electrolyte Disorders, Education In Self-Care In Life With Illness And Disability In Relation To The Sick Person And His Family, Education In Ensuring Appropriate Conditions For The Patients Environment At Home, Education In The Field Of A Healthy Lifestyle And Optimal Forms Of Life Activity2) Providing Instructions In The Field Of Basic Tasks And Ways Of Caring For The Sick Person, With Particular Emphasis On The Specificity Of The Disease Or Disability, Including: Prevention Of Complications Thrombosis, Proper Patient Care, Prevention Of Pressure Sores, Sores And Contractures, Instruction Of The Patient And His Caregiver In The Use Of Care/Rehabilitation Equipment And Medical Devices3) Consultations On Practical Solutions Facilitating Care And Nursing Activities4) Measuring, If Necessary, Blood Pressure, Blood Sugar Levels And Measuring Other Agreed Vital And Preventive Parameters. The Total Number Of Hours Allocated In The Project Is Approx. 360 Hours. Duration Of The Service - Minimum 1 Hour (I.E. 60 Minutes). It Is Planned To Support Approximately 16 People, But The Number Of Participants Is An Estimate And May Change During The Execution Of The Order. Intervention Visits Are Allowed If Necessary. The Target Group Will Be Qualified In Accordance With The Rules Of The Regulations On Recruitment And Participation In The Project Cus - New Look At Social Services.2.2.1. Terms Of Service: A) Form Of Engagement: Mandate Contract, B) Service Completion Date: From The Date Of Signing The Contract To December 31, 2027.2.2.2. In Addition To Working Directly With Project Participants, The Contractor Will Be Obliged To: A) Perform Its Tasks Diligently, Effectively And On Time.
