Tenders Are Invited For Supply Of Medical Devices (Cryoablation Probes) Along With The Rental Of A Cryoablation Apparatus

1. The Subject Of The Order Is The Delivery Of Medical Devices (Cryoablation Probes) Along With The Rental Of A Cryoablation Device To The Headquarters Of The Ordering Party (Kraków, Os. Złota Jesieni 1), In Accordance With The Description And Requirements Contained In Annex No. 1A (Concerning Medical Devices) And Annex No. 1B (Concerning The Cryoablation Device) To This Specification.2. The Validity Period/Warranty Period Of Medical Devices Cannot Be Shorter Than 12 Months From The Date Of Delivery. Requirements For Medical Devices:3. The Contractor Undertakes To Conduct, At The Request Of The Ordering Party, Training Of The Ordering Partys Staff At Its Registered Office (Os. Złota Jesieni 1, Kraków) Together With The First Delivery Of The Goods, I.E. Medical Products, To The Hospital Pharmacy Within The Time Agreed In Writing With The Ordering Party, But No Later Than 30 Days From The Date Of Conclusion Of The Contract.4. The Contractor Will Be Obliged To Submit An Attendance List From The Training Referred To In Section 3 With Signatures Of Trained Users Confirming Their Actual Presence To The Hospital Pharmacy, Within 5 Business Days From The Date Of The Training. 5. The Description Of The Subject Of The Order In The Field Of Medical Devices Is Included In Annex No. 1A To These Specifications. Requirements For The Device: 6. On The Day Of Delivery Of The Subject Of The Contract, I.E. The Cryoablation Device, The Contractor Is Obliged To Train The Ordering Partys Employees On The Delivered Devices At The Ordering Partys Headquarters (Złota Jesieni Os., 1 Kraków) Or To Provide Such Training At Its Own Expense By A Third Party, Which Will Be Confirmed By Issuing Personalized Certificates.7. The List Of Technical And Operational Parameters Limiting The Rental Of A Cryoablation Device Is Included In Annex No. 1B To Swz.8. The Remaining Terms Of The Order Are Specified In The Draft Contract Constituting Annex No. 3A (Concerning Medical Devices) And Annex No. 3B To The Swz (Concerning The Cryoablation Device).
