The Subject Of The Order Is Purchase Of A Comprehensive Service Of Organizing And Conducting A One-Day Conference For A Maximum Of 120 People In Radom As Part Of Cooperation At The Regional Level. The Order Is Financed Under Fem Measure 6.3 Of The European Funds For Mazovia 2021-2027 Program Of The Eures In The Masovian Region Regional 1 Project. The Description Of The Subject Of The Order - (Hereinafter Opz) Is Attached As Annex No. 2 To The Swz. Clauses Socialordering Party: 1. The Ordering Party Requires The Contractor To Employ, Under An Employment Contract, A Person To Perform The Activities Of A Contract Coordinator Throughout The Entire Order Execution Period, To The Detailed Extent Specified In The Contract Template.2. During The Execution Of The Order, The Ordering Party Will Be Entitled To Carry Out Inspection Activities Towards The Contractor In Terms Of Meeting The Requirement Indicated In Point 1.3. The Ordering Party Will Be Entitled In Particular To: 3.1) Request Declarations And Documents Confirming Compliance With The Requirement Referred To In Point 1 And Evaluate Them, In Particular: A) In Relation To The Person Referred To In Point 1 - Employment Contract And Other Documents Confirming The Performance Of Work, Including Documentation Related To Working Time And Scope Of Duties; 3.2) Request Explanations In Case Of Doubts As To The Fulfillment Of The Requirement Referred To In Point 1; 3.3) Carrying Out On-Site Inspections Of The Provision Of Services To The Extent Necessary To Confirm Compliance With The Requirement Referred To In Point 1.4. The Contractor Is Obliged, At Each Request Of The Ordering Party, Within The Specified Deadline, To Submit Evidence Confirming The Fulfillment Of The Requirement To Employ The Person Referred To In Point 1, In Particular: 4.1) The Contractors Declaration Of Employment Under An Employment Contract Of A Person Being The Coordinator Of The Contract. This Declaration Should Include: A Precise Description Of The Entity Submitting The Declaration, The Date Of Submission Of The Declaration, An Indication That The Activities Covered By The Request Are Performed By A Person Employed Under An Employment Contract, Along With The Name And Surname Of This Person, The Type Of Employment Contract And The Working Hours, As Well As The Signature Of The Person Authorized To Submit The Declaration On Behalf Of The Contractor; 4.2) A Copy Of The Contract Of The Person Who Coordinates The Contract, Certified By The Contractor As A True Copy Of The Original (Together With A Document Regulating The Scope Of Duties, If Prepared). A Copy Of The Contract Should Be Anonymized In A Way That Ensures The Protection Of Personal Data; The Employees Name And Surname Are Not Anonymized.
