The Subject Of The Order Is The Implementation Of A Task Called Provision Of Assistant Services At Home For Disabled People Living In The Area Of ​​The City Of Lubartów, The Lubartów Commune And The Kamionka Commune. In Particular:2. The Service Will Be Carried Out By 6 Assistants For 18 People With Disabilities For 8 Months For An Average Of 30 Hours Per Person, I.E. 18 X 30 Hours = 540 Hours Per Month, 540 Hours X 8 = 4,320 Hours In Total. One Assistant Spends 720 Hours Over An 8-Month Period, And Approximately 90 Hours Per Month. The Support Schedule Will Be Developed Depending On The Needs Of The Disabled Person. Partial Offers: The Subject Of The Order Has Been Divided Into The Following Parts: 1) Part I: Provision Of Assistant Services Until March 31, 2027; 2) Part Ii: Provision Of Assistant Services Until March 31, 2027; 3) Part Iii: Provision Of Assistant Services Until March 31, 2027; 4) Part Iv: Provision Of Assistant Services Until March 31, 2027 R.;5) Part V: Provision Of Assistant Services Until March 31, 2027;6) Part Vi: Provision Of Assistant Services Until March 31, 2027;3. The Contractor May Submit An Offer For All Parts Of The Order Indicated Above. The Ordering Party Does Not Limit The Number Of Parts For Which The Order May Be Awarded To The Same Contractor.4. The Scope Of Provided Assistant Services Includes In Particular:1. Support In Performing Daily Activities: 1) Assistance In Moving Around (Inside And Outside The Home); 2) Assistance In Dressing And Undressing; 3) Support In Eating Or Preparing Meals; 4) Assistance In Maintaining Personal Hygiene (In The Scope Not Covered By Medical Care).2. Support In Moving Outside The Place Of Residence: 1) Accompanying To Offices And Public Institutions; 2) Assistance In Visits To Doctors Or Other Health Care Facilities; 3) Assistance In Reaching Service And Commercial Points. 3. Support In Social Participation: 1) Accompanying In Cultural, Sports And Religious Events; 2) Assistance In Maintaining Social And Family Contacts; 3) Assistance In Participating In Activation And Integration Activities.4. Support In The Use Of Education And Professional Activity:1) Assistance In Reaching Educational Or Training Institutions;2) Assistance In Activities Related To Study Or Work (Including Remote Work).5. Support In Organizing Everyday Matters: 1) Assistance In Dealing With Official Matters; 2) Assistance In Using Social And Health Services; 3) Support In Planning And Organizing The Day.5. The Scope Of Assistant Services Will Be Determined Individually For Each Project Participant And Will Result From The Support Need Assessment Prepared For Each Project Participant.
