The Subject Of This Procedure Is The Delivery Of Medical Devices, I.E. 4 Pieces Of Electric Chairs - Self-Propelled Stair Climbers For The Independent Public Health Care Center Of The District Emergency Service In Sosnowiec. The Description Of The Subject Of The Order Is Annex No. 1A To Swz.2. Attachment No. 1A To This Tor, Which Is Also The Description Of The Subject Of The Order, Describes The Minimum Required Technical And Operational Parameters Of The Subject Of The Order.3. The Ordering Party Requires That The Subject Of The Order Be Brand New, In Complete Condition, Without The Need To Make Additional Deliveries Or Incur Costs, And Cannot Be The Subject Of Rights Or Claims Of Third Parties.4. The Subject Of The Order Should Come From Current Production, Be Free From Physical And Legal Defects, Of The Highest Quality And Admitted To Trading In The Eu. Moreover, No Element Of The Subject Of The Order Or Any Of Its Components May Be Reconditioned, Used For Exhibition Purposes Or Previously Used By Another Entity/Person For Any Purpose.5. The Offered Products Within The Scope Of The Subject Of The Order Must Have A Certificate Or Declaration Of Conformity Or Authorization Allowing Marketing And Use In Medical Entities, As Well As Appropriate Labeling In Accordance With The Law. 6. All Products Indicated In The Offer Must Be Admitted To Trading In The Eu. 7. The Ordering Party Requires Documentation Of The Subject Of The Order In Polish, Including Operating Instructions, Quality Guarantees, Technical Documentation, Certificates Or Declarations Of Conformity, Passports And Other Documents In Accordance With The Provisions Of Law. 8. The Contractor Is Responsible For The Quality Of The Entire Subject Of The Order And The Quality Of The Materials Used. 9. Delivery Of The Subject Of The Order Takes Place At The Expense And Risk Of The Contractor. 10. The Ordering Party Requires A Quality Guarantee On The Following Principles: For Medical Devices For A Minimum Period Of 24 Months Counted From The Date Of Signing The Acceptance Protocol Of The Subject Of The Order By The Ordering Party. During The Quality Guarantee Period, The Contractor Is Obliged To Remove Free Of Charge Any Failures, Faults And Defects Detected In The Subject Of The Order Or To Deliver Items Free From Defects, Within A Period Not Longer Than 7 Days From The Date Of Their Reporting. The Contractor Will Begin To Remove Faults, Failures Or Faults Within 24 Hours From The Moment It Receives The Notification, Excluding Public Holidays. At The Request Of The Ordering Party, The Contractor Will Provide A Replacement Medical Device.11. The Order In Question Is Co-Financed In 85% By The European Funds For Silesia Program 2021-2027, Co-Financed By The European Social Fund Plus.
