Tenders Are Invited For Purchase And Delivery Of Medical Equipment And Supplies Under The Grant Project Called Accessibility Plus For Aos Divided Into Parts

1. The Task Includes Delivery, Transport, Bringing And Assembly (Installation) Of The Devices Indicated In The Form To The Registered Office Of The Ordering Party And To The Indicated Rooms. Staff Training Is Provided For Selected Equipment (Indicated In The Specification Tables).2. The Subject Of The Order Is Divided Into Parts According To The Table Below: 1) Part 1 - Electric Couches3. The Equipment Must Be Brand New, Unused, And Should Have The Documents Required By Applicable Law. 4. The Contractor Declares That It Meets The Conditions Specified In Separate Regulations Necessary To Perform The Contract, Has Sufficient Experience And Qualifications To Perform The Contract And Undertakes To Properly Perform The Subject Of This Contract.5. The Contractor Declares That The Goods Indicated In All Assortment Items Listed In The Assortment And Price Form Have The Appropriate Documents Allowing For Marketing, In Accordance With The Requirements Of The Current Legal Status, And That During The Term Of The Contract, He Will Constantly Monitor The Legal Status Of Admission To Trading In The Territory Of The Republic Of Poland. 6. The Contractor Declares That The Offered Equipment Complies With The Relevant Standards And Legal Provisions, In Particular With Regard To Admission To Trading And Use, In Accordance With The Act Of 7 April 2022 On Medical Devices - In Order To Prove This, The Contractor Will Present, At The Request Of The Ordering Party, Appropriate Documents Within 5 Days Of Submitting The Request (In The Scope Of Goods Being A Medical Device).7. Each Batch Of Delivered Goods Should Be Accompanied By The Necessary Information - Labels And Instructions For Use In Polish.8. The Contractor Completes The Attachment Only In The Scope Of The Task For Which He Is Submitting An Offer. The Remaining Scope - Deletes/Deletes, Etc., In A Way That Will Not Raise Doubts As To The Scope For Which The Contractor Submits An Offer.9. The Description Of The Subject Of The Order Was Prepared In Connection With The Implementation Of The Grant Agreement And Based On The Guidelines And Descriptions Contained In The Documents Applicable At The Application Submission Stage, In Particular In The Application Recruitment Procedure And In Annex No. 1 To The Aos Accessibility Standard - Description Of Properties For Patients With Special Needs, Including Those With Disabilities, Which Indicate How To Implement Accessibility For People With Special Needs In Medical Facilities. The Description Of The Subject Of The Order Takes Into Account The Requirements And Recommendations Resulting From The Report On The Initial Audit Of The Facility, Including The Requirements Of The Aos Accessibility Standard Indicated Therein, Recommended For Implementation Under The Project, In Particular The Key Tasks.
