1) The Subject Of The Order Is The Provision Of A Service Consisting In The Collection, Transport And Disposal Of Medical Waste From Its Storage Place, Located At The Municipal Hospital In Rabka - Zdrój Sp. Z O. O., With An Average Of Three Collections A Week. The Contractor Is Obliged To Provide The Service With Its Own Transport And Ensure Loading Of Waste Onto The Means Of Transport.2) The Scope Of The Order Includes The Collection, Transport And Disposal Of Medical Waste Classified In Accordance With The Waste Catalog Specified In The Regulations Issued On The Basis Of The Waste Act, In The Following Codes: Waste Code Type Of Waste18 01 02* Body Parts And Organs As Well As Containers For Blood And Preservatives Used For Its Storage18 01 03* Other Waste That Contains Live Microorganisms Pathogens Or Their Toxins And Other Forms Capable Of Transferring Genetic Material Known Or For Which There Are Reliable Grounds For Believing That They Cause Diseases In Humans And Animals (E.G. Infected Diapers, Panties, Sanitary Pads, Pads), Excluding 18 01 80 And 18 01 82, I.E. E.G. Waste Contaminated With Blood, Secretions Containing Pathogenic Factors Or Containing Blood In Liquid Form - Contaminated Vessels, Dressings, Gauze Pads, Used Catheters, Surgical And Treatment Tools, Syringes, Laboratory Glassware, Drip Devices, Medical Spatulas18 01 04 Waste Other Than Those Mentioned In 18 01 03, I.E. Non-Infectious Waste - Surgical Tools Without Sharp Objects, Dressings, Including Plaster Casts, Syringes (Without Needles), Infusion Bottles And Bags, Bowls, Dishes, Disposable Clothing Use, Respiratory Protection Masks, Gloves, Disposable Articles).18 01 06* Chemicals Containing Dangerous Substances18 01 82* Residues From Feeding Infectious Patients 18 01 01 Surgical Instruments And Their Remains 18 01 07 Chemicals Other Than Those Mentioned In 18 01 06 18 01 09 Medicines Other Than In 18 01 0818 01 80 Used Peloids After Treatments Performed As Part Of Medical Activities With Infectious Properties3) The Expected Amount Of Waste To Be Collected, Transported And Disposed Of Within A Period Of 24 Months Is Specified In Annex No. 1 To The Swz - Offer Form. The Quantities Provided Are Estimates And Are Used Only To Calculate The Offer Price And Compare Offers.4) The Actual Amount Of Waste Sent For Collection During The Period Of The Order Will Depend On The Current Needs Of The Ordering Party, In Particular On The Number Of Hospitalized Patients And The Scope Of Health Services Provided. Therefore, The Ordering Party Reserves The Right To Reduce Or Increase The Amount Of Waste In Relation To The Estimated Quantities, Subject To The Provisions Of The Contract And The Provisions Of The Public Procurement Law.5) Reducing The Scope Of The Order Execution In Relation To The Estimated Quantities Does Not Constitute A Basis For The Contractor To Pursue Any Claims Against The Ordering Party, Provided That The Ordering Party Completes The Order With A Gross Value Constitutin
