5.1 Name Of The Order: Reconstruction Of Ul. Inwałdzka In Zagórnik 5.2 The Subject Of The Order Is The Execution Of The Task Entitled: Reconstruction Of Ul. Inwałdzka In Zagórnik In Accordance With The Conditions Specified In The Swz. The Scope Of The Order Includes In Particular The Execution Of: 5.2.1 Demolition Works, 5.2.2 Felling And Planting Trees, 5.2.3 Earthworks, 5.2.4 Road Construction, 5.2.5 Sidewalk, Roadsides, Exits, Parking Spaces And Road Elements, 5.2.6 Drainage And Drainage Elements, 5.2.7 Demolition And Construction Of Culverts, 5.2.8 Rainwater Drainage, 5.2.9 Technological Channel, 5.2.10 Securing Underground Networks And Regulating And Replacing Manholes Of Underground Equipment, 5.2.11 Markings And Traffic Safety Elements, 5.2.12 Finishing Works, 5.2.13 Reconstruction Of The Power Grid And Lighting, 5.2.14 Performance Of Other Works And Works Necessary For The Proper Performance Of The Subject Of The Contract. 5.3 The Description Of The Subject Of The Order Is Included In The Annexes To The Swz, I.E.: Annex No. 3 And 3.1 Design Documentation, 3.2 Technical Specification For The Execution And Acceptance Of Works, 3.3 Bill Of Quantities. The Contractor Is Obliged To Complete The Order On The Terms And Conditions Described In The Draft Contract. 5.4 The Ordering Party Requires Pursuant To Art. 95 Section 1 Of The Public Procurement Law, Employment Under An Employment Contract In Ones Own Enterprise Or By A Subcontractor Of Persons Performing Activities Specified By The Ordering Party In The Scope Of Contract Execution, If The Performance Of These Activities Involves Performing Work In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974, Labor Code, I.E.: Performance Of Demolition Works, Felling And Planting Trees, Earthworks, Road Construction, Sidewalk, Roadsides, Exits, Parking Spaces And Road Elements, Drainage And Drainage Elements, Demolition And Construction Of Culverts, Rainwater Drainage, Technological Channel, Protection Of Underground Networks And Regulation And Replacement Of Equipment Manholes Underground, Signage And Traffic Safety Elements, Finishing Works, Reconstruction Of The Power Grid And Lighting. 5.4.1 The Contractor Will Be Obliged To Submit To The Ordering Party, Upon Each Request And Within The Deadline Specified In The Request, The Following Evidence Confirming The Fulfillment Of The Requirement Of Employment Under An Employment Contract By The Contractor Or Subcontractor Of Persons Performing The Indicated Activities During The Execution Of The Order: A) Declaration Of The Contractor Or Subcontractor On The Employment Under An Employment Contract Of Persons Performing The Activities Covered By The Request Of The Ordering Party.
