The Service Titled Rehabilitation For Seniors Will Be Implemented In The Period From The Date Of Signing The Contract In 2026 To September 30, 2028 And Will Be Addressed To Residents Of The Kotlin Commune Aged 60+. The Price Of The Mobile Rehabilitation Service Includes Assistance To People Aged 60+ Living In The Kotlin Commune. The Service Should Be Understood As: Assistance To People After Injuries, Surgeries Or Chronic Pain, As Well As Physiotherapy And Manual Therapy, As Well As Treatments That Should Be Performed At The Patients Home Without The Need To Visit A Medical Facility. The Service Will Be Settled On The Basis Of An Invoice Or Bill Issued In Accordance With The Provisions Of The Contract (Appendix No. 4 To The Swz), Depending On The Selected Offer. Regulations Have Been Prepared For The Rehabilitation For Seniors Service, Which Are An Integral Part Of The Contract. Support Will Be Provided Directly At The Participants Place Of Residence, Which Will Enable Seniors To Benefit From Rehabilitation Also For People With Limited Mobility. The Subject Of The Order Are Individual Rehabilitation Services For People 60+ Living In The Kotlin Commune After 8 Hours. Treatments Per 1 Person. In 2026, 256 Services = 32 People In 2027, 360 Services = 45 People In 2028, 264 Services = 33 People The Minimum Requirements Regarding The Subject Of The Order Are Described In Annex No. 12 To Swz - Description Of The Subject Of The Order (Opz)
