The Subject Of The Order Is To Provide Transportation Of A Disabled Child By Medical Transport Along With The Provision Of Care During Transport To The Educational And Therapeutic Center Iskierka Rehabilitation And Education Center In Grybów In The School Year 2026/2027.2. The Expected Number Of People Covered By Transport In The 2026/2027 School Year Is 1.3. Medical Transport Should Be Carried Out Using A Technically Efficient, Specialized Means Of Sanitary Transport - A Type B Ambulance, In Accordance With The Pn En 1789 Standard, Designed To Transport A Person In A Lying Position. 4. The Driver Must Be Authorized To Drive Emergency Vehicles Within The Meaning Of Art. 106 Of The Act Of January 5, 2011 On Vehicle Drivers (Consolidated Text: Journal Of Laws Of 2025, Item 1226, As Amended).5. The Childs Guardian May Be A Nurse With The Qualifications And Authorizations Required For Nurses Or A Paramedic.6. The Driver Of A Vehicle Cannot Also Be A Guardian.7. Due To The Childs Epileptic Seizures, It May Be Necessary For The Caregiver To Administer A Relaxing Drug Provided By The Childs Parent During Transport.8. The Driver And Guardian Are Obliged To Bring The Child In And Out Of The Vehicle.9. Transportation Will Be Provided Every Day, Except Public Holidays And Holidays In The 2026/2027 School Year, Using A Vehicle Adapted To Transport Disabled Children.10. Transportation Will Be Provided In Accordance With The Timetable Developed Together With The Ordering Party And Adapted To The Schedule Of The Child Using The Transportation. Subsequent Changes To The Timetable Require The Acceptance Of The Ordering Party.11. The Ordering Party Reserves The Right To Change The Scope Of The Order Resulting From Organizational Reasons.12. The Order Includes Transport Of The Child To The Iskierka Educational And Therapeutic Center, Rehabilitation And Educational Center In Grybów, Together With The Childs Return Transport To The Place Of Residence.13. The Service Provider Is To Ensure Punctual, Safe, Efficient And Failure-Free Delivery. The Child Must Be Brought To The Center No Earlier Than 20 Minutes Before The Start Of Classes And No Later Than 10 Minutes Before The Start Of Classes.14. In The Event Of A Breakdown Of The Designated Vehicle, The Contractor Will Provide A Replacement Vehicle With The Same Parameters, I.E. A Sanitary Vehicle - A Type B Ambulance, In Accordance With The Pn En 1789 Standard, Designed To Transport A Person In A Lying Position.15. In Winter, The Vehicle Transporting The Child Must Be Heated.16. The Carrier Insures The Vehicle And Passengers Against Any Damage Incurred During And In Connection With The Transport.17. Settlement Of Delivery Will Be Made Monthly On The Basis Of A Correctly Issued Invoice/Bill. 18. The Order Completion Date Is 01/09/2026-31/08/2027.19. The Ordering Party Allows For The Possibility Of Making Changes Resulting From The Provisions Of The Concluded Contract, Including Changes In The Value Of The Contract Fo
