The Ordering Party Requires That The Execution Of The Subject Of The Order Take Place On The Terms And Conditions Specified In The Detailed Description Of The Terms Of Service Implementation Constituting Annexes No. 2 And 2.1 To 2.7 And Annexes No. 3 And 3.1 To 3.2, As Well As In The Contract Template Constituting Annex No. 4 To The Swz. The Ordering Party Requires That The Offer Be Attached As Evidence Of A List Of All Disinfectants, Cleaning And Disinfecting Agents, Washing Agents, Cleaning Agents, Preservative Agents, Etc. Necessary For The Proper Implementation Of The Subject Of The Order. The Ordering Party Requires The Following: Safety Data Sheet, Permission To Trade Biocidal Preparations In The Eu And The Republic Of Poland, Microbiological Effectiveness Tests And Catalog Cards - Leaflets. The Contractor Will Attach A Statement In Which He Will Certify That The Means Used To Provide The Service (Reusable Cloths, Mops) Are Delivered To An External Laundry With A Hygienic Barrier And A Positive Opinion Of The State District Sanitary Inspector. 2. If The Contractor Fails To Submit The Evidence Or The Evidence Submitted Is Incomplete, The Ordering Party Calls For Its Submission Or Completion Within The Specified Deadline. 3. The Provisions Of Section 2 Shall Not Apply If The Evidence In Question Serves To Confirm Compliance With The Features Or Criteria Specified In The Description Of The Offer Evaluation Criteria Or, Despite The Submission Of The Evidence In Question, The Offer Is Rejected Or There Are Grounds For Invalidation Of The Proceedings. 4.
