I. Subject Matter Of The Order The Subject Of The Order Are Construction Works Under The Task Titled: Modernization Of Cold Distribution In Building E Of The Provincial Hospital In Tarnobrzeg. 1. Cpv Code According To The Common Procurement Vocabulary: 71220000-6 Architectural Design Services 71320000-7 Engineering Services In The Field Of Design 2. The Subject Of The Order Includes: 1) Preparation Of Documentation In The Field Of Concept Development, Construction Design, Technical Design, Executive Design, Along With Bills Of Quantities And Investor Cost Estimates, Technical Specifications For The Execution And Acceptance Of Works (Stwior), Including: A) Modernization Hospital Pharmacy Located On The Second Floor Of Building F1 B) Modernization Of The Pharmacy Warehouse Located In The Basement Of Building F1 C) Modernization Of The Cytostatic Drugs Laboratory D) Creation Of The Nutrition Laboratory 2) Obtaining A Decision On Development Conditions And A Final Building Permit Decision As Well As Other Necessary Arrangements, Tests, Expert Opinions And Analyzes Necessary For The Proper Execution Of The Subject Of The Order, Including: A) Approval By The Sanitary And Epidemiological Station Or A Sanitary Appraiser B) Agreement With Fire Protection Appraiser C) Agreement With The Pharmaceutical Inspectorate D) Excavation Of Flat Roof Layers, Examination Of Internal Humidity And Technical Opinion On Its Technical Condition 3) Performing Authors Supervision. 3. A Detailed Description Of The Subject Of The Contract Is Included In Annex No. 1 To The Swz - Description Of The Subject Of The Contract. 4. The Rules For The Implementation Of The Service In Question Are Specified In The Draft Agreement Constituting Annex No. 8 To The Swz. 5. If The Description Of The Subject Of The Contract Refers To Standards, European Technical Assessments, Approvals, Technical Specifications And Technical Reference Systems Referred To In Art. 101 Section 1 Point 2 And Section 3 Of The Act, The Ordering Party Indicates That It Allows Equivalent Solutions, And Such A Reference Is Accompanied By The Words Or Equivalent. 6. If, In The Description Of The Subject Of The Order, The Ordering Party Indicated Trademarks, Patents Or Origin, Sources Or A Specific Process That Characterizes The Products Or Services Provided By A Specific Contractor, In This Respect The Ordering Party Allows The Use Of Equivalent Products, Devices And Goods. 7. The Contractor Who Envisages Using Products, Devices And Goods Equivalent To Those Described By The Ordering Party Is Obliged To Demonstrate That The Offered Products, Devices And Goods Are Equivalent Because They Meet The Requirements Specified By The Ordering Party. The Burden Of Demonstrating This Compliance Rests With The Contractor. In Order To Assess The Equivalence Of The Products, Equipment And Goods Offered By The Contractor, The Contractor Will Attach Their Description To The Offer, Showing The Equivalence Features.
