Tenders Are Invited For Wraparound Support Service

Note: This Opportunity Is Only Available To Providers Who Have Already Been Awarded A Place On Peterborough City Council'S Care In The Community Pdps. Any Expressions Of Interest, Smt Or Tender Submissions From Organisations Who Have Not Been Awarded A Place On The Pdps Will Be Rejected.Peterborough City Council Will Be Utilising Lot 6 Of The Care In The Community Pdps To Procure A Wraparound Support Service.This New Service Will:•Respond To People In Crisis, Providing Short-Term Support While Building Long-Term Capacity•Improve The Lives Of Residents, Increasing Or Rebuilding Their Independence•Provide Immediate, Short-Term Support To People Whose Carer Is Unable To Continue Providing Support Due To An Emergency.•Facilitate Earlier Hospital Discharge And Prevent Repetitive/Unnecessary Admissions•Prevent Or Reduce The Cost Of Care Placements•Align With Pcc’S Existing Reablement Offer, Supporting Those Who May Not Meet The Reablement Service’S Current Criteria Due To The Complexity Of Their Care Needs.The Service Will Consist Of Two Elements:-Short-Term Outcome-Based Care Provision-Specialist Interventions To Build Long-Term Resilience.The Service Will Be Delivered In Collaboration By Three Home Care Providers. Each Will Recruit And Train Specialist Care Staff To Deliver Both Elements. Depending On Need Support Will Either Be Delivered At Home Or In An Extra-Care Setting. This Is A Pilot, With The Contract Term Being 12 Months, With The Option To Extend Twice For Further Periods Of 6 Months (24 Month Term In Total)
