The Service Is For The Provision Of An Nhs Community Health Check Programme To Be Delivered By A Single Provider. Please Note This Is To Compliment The Service That Is Being Delivered By Gp'S And Is Primarily Focussed At Capturing People Who May Not Otherwise Attend A Gp Clinic Whilst Out And About Within The Community.Health Checks Is A 5-Yearly Contract With Health Care For Adults Aged 40-74 Without A Diagnosis Of Heart Disease, Kidney Disease, Stroke, Or Type 2 Diabetes. Residents Will Receive A Health Check Every 5 Years To Manage Cardiovascular Disease Risk Factors (Cvdrfs) For Those Conditions.
