Wspólna In Sułkowice, At Km 0+000 - 0+432 In Accordance With The Conditions Specified In The Swz, Including In Particular: 5.2.1 Modernization Of The Access Road To Agricultural Land At Km 0+102-0+432, Within The Plot No. 2111/2 In Sułkowice, 5.2.2 Modernization Of The Road At Km 0+000-0+102 And At Km 0+102-0+432 Outside Plot No. 2111/2 In Sułkowice, 5.2.3 Performance Of Other Works And Works Necessary For The Proper Performance Of The Subject Of The Contract. 5.3 The Description Of The Subject Of The Order Is Included In The Annexes To The Swz, I.E.: Annex No. 3 And 3.1 Design Documentation, 3.2 Technical Specification For The Execution And Acceptance Of Works, 3.3 Bill Of Quantities. The Contractor Is Obliged To Complete The Order On The Terms And Conditions Described In The Draft Contract. 5.4 The Ordering Party Requires Pursuant To Art. 95 Section 1 Of The Public Procurement Law, Employment Under An Employment Contract In Ones Own Enterprise Or By A Subcontractor Of Persons Performing Activities Specified By The Ordering Party In The Scope Of Contract Execution, If The Performance Of These Activities Involves Performing Work In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974, Labor Code, I.E.: Preparatory And Demolition Works, Earthworks, Renovation Of Roads, Roadsides And Drainage, Finishing Works. 5.4.1 The Contractor Will Be Obliged To Submit To The Ordering Party, Upon Each Request And Within The Deadline Specified In The Request, The Following Evidence Confirming The Fulfillment Of The Requirement Of Employment Under An Employment Contract By The Contractor Or Subcontractor Of Persons Performing The Indicated Activities During The Execution Of The Order: A) Declaration Of The Contractor Or Subcontractor On The Employment Under An Employment Contract Of Persons Performing The Activities Covered By The Request Of The Ordering Party.
