The Subject Of The Order Is The Provision Of Services Consisting In Providing Access To Sports And Recreational Facilities And Activities For Employees Of Courts In The Łódź District And Members Of Their Families And Other Registered Persons (Hereinafter Also Referred To As Participants), As Part Of A Monthly Subscription, Consisting In Providing Access To Various Sports And Recreational Facilities And Activities As Part Of The Sports And Recreation Program, Divided Into: 1) Unlimited Program - Package 1 For: - Court Of Appeal In Łódź - District Court In Łódź − District Court In Brzeziny − District Court In Łęczyca − District Court In Rawa Mazowiecka − District Court In Łowicz − District Court For Łódź - Śródmieście In Łódź − District Court For Łódź - Widzew In Łódź − District Court In Pabianice − District Court In Zgierz 2) Program Limited – Package 2 Only For: − Court Of Appeal In Łódź. The Services That Are The Subject Of The Order Consist In Enabling Participants Of The Unlimited Program (I.E. Giving The Opportunity To Use Various Recreational And Sports Activities In Various Sports And Recreational Facilities On The Same Day, Week Or Month And Without Declaration Of Use Of A Specific Location, And Without Additional Payments) And Limited Program (I.E. 8 Entries Per Month For One Participant From The Court Of Appeal In Łódź) - Access To Sports And Recreational Facilities Throughout Poland, And In In Particular In Łódź And The Łódź Voivodeship, Of Various Types As Part Of A Monthly Subscription. The Estimated Estimated Number Of People Interested In Participating In The Unlimited Program - Package 1 During One Month Is Approximately: • 1,037 Cards For Employees; • 474 Cards For Accompanying Persons; • 136 Pool Cards For Children Up To 15 Years Old; • 90 Full Cards For Children Up To 15 Years Of Age, Which Enable The Use Of All Available Services For Children Up To 15 Years Of Age; • 93 Full Cards For Children And Teenagers Aged 15 To 18. The Estimated Number Of People Interested In Participating In The Limited Program - Package 2 During One Month - Only For Employees Of The Court Of Appeal In Łódź Is Approximately: • 12 Cards For Employees Of The Court Of Appeal In Łódź. The Ordering Party Reserves The Right To Make Changes To The Quantity Of Ordered Cards. The Ordering Party Will Be Entitled To Purchase A Larger Or Smaller Number Of Cards In Individual Variants, While Maintaining The Contract Amount And Unit Prices Of Individual Cards.
