The Subject Of The Order Is The Successive Delivery Of Sodium Hypochlorite - Sodium Chlorate I In The Amount Of 595.2 Tons (Hereinafter Referred To As The Product) Used For Disinfection Of Water Intended For Human Consumption, Transported By The Contractor To The Goczalkowice Water Treatment Plant, Ul. Jeziorna 5, 43-230 Goczalkowice. 2. A Detailed Description Of The Subject Of The Order (Opz) Is Included In Annex No. 5 To The Swz. 3. Detailed Conditions For The Execution Of The Order Are Set Out In Annex No. 6 To The Swz. 4. The Means Of Proof In Question: 4.1. In Order To Demonstrate That The Offered Product Complies With The Requirements Specified In Annex No. 5 To The Swz - Opz, The Ordering Party Requests That The Contractor Submit The Following Evidence Together With The Offer: 1) A Current Hygienic Certificate Issued By The National Institute Of Public Health - National Institute Of Hygiene, Authorizing The Product For Use In Water Intended For Human Consumption; 2) Current Marketing Permit For A Biocidal Product - Sodium Hypochlorite - Sodium Chlorate I Issued On The Basis Of The Act Of October 9, 2015 On Biocidal Products (Consolidated Text: Journal Of Laws Of 2021, Item 24); 3) Technical Specification Of The Offered Product. Note: The Technical Specification Should Confirm Compliance With All Requirements Specified In Point 1 Of Annex 5 To The Swz - I.E. In The Product Characteristics. The Ordering Party Will Accept Equivalent Means Of Evidence If They Confirm That The Offered Deliveries Meet The Requirements Specified By The Ordering Party. Note 2: As Part Of The Technical Specification Of The Offered Product, The Ordering Party Requires Specific Numerical Values (Does Not Apply To The Form Requirement), Confirming The Fulfillment Of The Order Conditions Specified In Point 1 Of The Opz (Product Characteristics). Note 3: The Ordering Party Requires That The Evidence Presented Clearly Shows That It Concerns Sodium Hypochlorite. 4.2. The Ordering Party Stipulates That If The Contractor Fails To Submit The Evidence Referred To Above, Or If The Evidence Submitted Turns Out To Be Incomplete, The Contractor Will Be Summoned In Accordance With Art. 107 Section 2 Of The Act To Submit Or Supplement Them Within The Prescribed Deadline.
