The Subject Of The Order Is: Preparation Of Complete Design And Cost Estimate Documentation For The Public Utility Building At Grochowska 259 A In The Form Of: Preparation Of Multi-Sector Design And Cost Estimate Documentation For The Reconstruction (Revitalization) With An Extension Of 1 (One) Floor, Modification Of The Functional Layout, Reconstruction Of Staircases With Retrofitting The Facility With Elevators, Thermal Modernization And Development Of The Adjacent Area, Utility Building At Ul. Grochowska 259A In Warsaw. 2. Currently, The Building Houses Care And Day Care Facilities (Including Community Self-Help Homes For People With Intellectual Disabilities And Chronic Mental Disorders). The Facility Is Intended For Daily Stay Of Residents. Due To The Specific Business Profile And The Care And Therapeutic Nature Of The Building, The Planned Modernization And Extension Must Absolutely Take Into Account The Highest Standards Of Universal Design. The Modernized Facility Must Be Rigorously Adapted To The Needs Of People With Special Needs (Including Multiple Disabilities), Ensuring Full Fire Safety, Acoustic Comfort And Appropriate Sanitary And Hygienic Conditions. 3. The Scope Of The Order Includes The Preparation Of All Studies, Analyses, Designs And Documents Necessary To Prepare The Investment, Including Obtaining The Required Arrangements, Opinions And Administrative Decisions, Removing Identified Defects, Irregularities And Functional Limitations Of The Facility, As Well As Adapting It To A New Utility Function, While Respecting Its Historical And Architectural Values. 4. The Documentation Must Be Complete From The Point Of View Of The Purpose It Is To Serve, I.E. Enable The Conduct Of The Public Procurement Procedure For Construction Works, Implementation Of Construction Works And Achievement Of The Expected Technical, Energy, Functional And Aesthetic Effect, Including Enabling The Contractor To Prepare A Complete Description Of The Subject Of The Construction Works Contract In Accordance With The Public Procurement Law. 5. The Detailed Scope Of The Order For Stages I-Iii Is Specified In The Contract Template Together With The Description Of The Subject Of The Order And Its Annexes, Which Constitutes Annex No. 4 To The Swz. 6. The Scope Of The Order Also Includes Stage Iv: Participation Of The Designer In The Tender Procedure On The Part Of The Ordering Party, For Performing Authors Supervision And For Preparation Of Energy Performance Certificates For Each Building Will Be Signed No Later Than Within 3 Days From The Date Of Initiation Of The Public Procurement Procedure For The Contractor Of Construction Works, Which Contains Detailed Requirements For The Implementation Of Stage Iv, Which Constitutes Annex No. 5 To The Swz. This Agreement Will Constitute 10% Of The Gross Value Of The Offered Gross Lump Sum Price Indicated In The Offer Form.
