1. The Subject Of The Order Is To Provide Veterinary Care For: 1) All Animals Registered In The Shelter For Homeless Animals Located In Bytom At Ul. Łaszczyka 18, 2) Animals Newly Admitted To The Shelter Due To Interventions Related To The Removal Of Animals From The Owner, 3) Animals Caught From The Bytom Commune That Escaped, Got Lost Or Were Abandoned. The Subject Of The Order Is Also The Contractors Provision Of Veterinary Assistance To Animals (Dogs And Cats) Injured In Traffic Accidents In The Bytom Commune. A Detailed Description Of The Subject Of The Order Is Included In Annex No. 6 To Swz.2. The Contractor Will Be Obliged To Provide Access To An Animal Medical Facility, Within The Meaning Of The Act Of December 18, 2003 On Animal Medical Facilities, Where Veterinary Care Will Be Provided 24 Hours A Day: 1) On Working Days; The Facility Should Be Located At A Distance Of No More Than 30 Km From The Shelter (Distance According To Indications After Entering The Location In Google Maps, According To The Shortest Route) - See: § 2 Section 2 Point 2) Of The Proposed Contractual Provisions. Distance Above The Facility From The Shelter Is One Of The Criteria For Evaluating Offers.2) On Saturdays, Sundays And Public Holidays And At Times When It Is Impossible To Provide Services At The Facility Indicated In Point 1) Above - Applies To Emergency Cases (After Traffic Accidents Or With Injuries Or Symptoms That Could Threaten The Life Or Health Of The Animal, As Well As In Cases Where Euthanasia Of The Animal Is Necessary); The Facility Should Be Located At A Distance Of No More Than 30 Km From The Shelter (Distance According To Indications After Entering The Location In Google Maps, According To The Shortest Route) - See: § 2 Section 2 Point 3) Of The Proposed Contractual Provisions.3. The Contractor Is Obliged To Employ, On The Basis Of An Employment Contract, All Persons Performing All Work In The Field Of Veterinary Care That Does Not Require The Qualifications Of A Veterinarian. Above The Obligation Does Not Apply To Entrepreneurs Personally Performing Any Of The Above. Works, As Well As Incumbent Members Of The Contractors Management Or Supervisory Bodies, Partners Of A General Partnership Or Partnership To The Extent That They Will Personally Perform The Service For The Ordering Party Or The Contractor. The Above Applies Accordingly To Subcontractors. Determining The Method Of Documenting The Employment Of These People, The Employers Rights To Control Compliance And Sanctions For Failure To Meet These Requirements Are Specified In The Draft Contractual Provisions. 4. The Ordering Party Does Not Reserve The Obligation For The Contractor To Personally Perform The Key Part Of The Order. Entrusting The Performance Of Part Of The Order To Subcontractors Does Not Release The Contractor From Responsibility For The Proper Performance Of This Order.5. The Ordering Party Does Not Allow The Possibility Of Submitting Partial Offers. The Ordering Party Di
