The Subject Of The Order Is The Provision Of A Transport Service Called Cus Heart On Wheels - Social Taxi In The Project: Lesser Poland Social Service Centers. The Transport Service Is Provided On The Basis Of Individual Applications Confirmed By The Coordinator Of Individual Social Services (Kipus) Or The Director Of Cus - Social Services Organizer Or Other Designated Cus Employee. The Transport Service Will Be Provided In An Amount Not Exceeding 6,000 Km Per Year In 2026. The Transport Service Includes, Among Others: Transporting The Recipient Of The Service From The Place Of Residence To The Destination, As Well As Return Transport To The Place Of Residence Within The Administrative Borders Of The Nowy Sącz And Gorlice Poviats In Order To Reach: Health Facilities (In Particular: Clinic, Pharmacy, Rehabilitation), Administrative Facilities (Taking Care Of Official Matters), Cultural, Social And Commercial Facilities (In Particular: A Grocery Store Or Other Point With Basic Necessities). A Person With The Need For Mobility Support Who Has Difficulties In Moving Independently By Public Transport May Use The Transport Service Together With One Guardian Or One Assistant Of A Person With Disabilities Or Persons Dependent On The Recipient Of The Service.
