The Subject Of The Order Is The Provision Of Transport Services Consisting In Daily Transport And Pick-Up Of Participants Of Occupational Therapy Workshops From Their Place Of Residence To The Premises Of The Workshops And Back. A) Transport Of Participants To Classes. Transport Of People With Disabilities To The Premises Of The Occupational Therapy Workshops In Trzebnica At Ul. Daszyńskiego 44–46 Must Be Completed Every Day No Later Than 8:30. Transport Will Be Carried Out From The Following Towns (Number Of Participants In Brackets): Oborniki Śląskie (2), Szewce (3), Strzeszów (1), Będkowo (1), Blizocin (1), Kuźniczysko (1), Masłowiec (1), Brzyków (1), Głuchów Górny (2), Piotrkowiczki (1), Małuszyn (1), Kaszyce Wielkie (1), Rościsławice (2), Skokowa (1),Szczytkowice (1),Sanie (1),Czachowo (1). A Detailed Timetable Will Be Developed By The Contractor In Consultation With The Ordering Party. The Contractor Is Obliged To Have At Least Three Vehicles Adapted To Transport People With Disabilities, Enabling The Transport Of A Folded Wheelchair. B) Transport Of Participants After Classes. Transport Of Participants After Classes Will Start At 2:30 P.M. From The Seat Of The Occupational Therapy Workshops In Trzebnica To The Towns Indicated In Point A), In Accordance With The Timetable Developed By The Contractor And Approved By The Ordering Party. C) Maximum Travel Time - The Participants Transport Time Cannot Exceed 1 Hour One Way. D) Transport Deadline The Transport Service Will Be Provided From Monday To Friday, Excluding Public Holidays And Other Days Agreed Between The Ordering Party And The Ordering Party. Contractor.E) Obligations Of The Driver The Driver Is Responsible For The Safety Of Transported Participants During The Journey And Is Obliged To Provide Assistance In Getting Into And Out Of The Vehicle.F) Transport Routes And Kilometer Limit Each Transport Route Requires Agreement And Acceptance Of The Ordering Party Before Concluding The Contract. The Total Daily Mileage Limit For All Vehicles Should Not Exceed 413 Km. This Limit Depends On The Actual Number Of Participants Present At The Occupational Therapy Workshops On A Given Day. When Calculating The Length Of Transport Routes, The Shortest Possible Travel Route With Participants Should Be Taken Into Account, Counted From The First Stop.G) Location Of Stops. Vehicle Stopping Places Are Determined By The Ordering Party In Consultation With The Contractor.H) Transport Of Guardians In Justified Cases, Transport May Be Ad Hoc Combined With Free Transport Of Able-Bodied Persons, In Particular Guardians Of People With Disabilities.I) Changes To Transport Routes During The Term Of The Contract, The Ordering Party Reserves The Right To Modifications To Transport Routes Resulting From Changes In The Number Of Participants Of Occupational Therapy Workshops Or Changes In Their Place Of Residence. These Changes Will Be Made Within The Maximum Daily Kilometer Limit Specified In Point F).
