This Specification Is For The Provision Of Minor Adaptations (Small-Scale Modifications Made To A Home) Which Are Crucial In Enabling Timely Discharge From Hospital, Supporting Post-Discharge Recovery, Preventing Falls, Preventing Admissions To Care Homes And Hospitals, And Maintaining Independence.
