The Subject Of The Order Is The Provision Of Shelter Services For 3 Homeless Men, With The Possibility Of Increasing The Number Of People Referred By 1, From The City Of Starogard Gdański In The Field Of Social Assistance Consisting In Providing Temporary Shelter And Other Services In Accordance With Art. 48 And 48A Of The Act Of March 12, 2004 On Social Assistance (Consolidated Text: Journal Of Laws Of 2025, Item 1214, As Amended).2. The Shelter Cannot Be Located Further Than 150 Km From The Ordering Partys Headquarters. The Distance Should Be Verified Via The Website Https://Www.Google.Pl/Maps Using The Route Function. By Distance, The Ordering Party Understands The Number Of Kilometers That Must Be Covered Using Public Roads, And Not A Straight Line Between The Ordering Partys Headquarters And The Shelter.3. The Ordering Party Stipulates That The Given Number Of People Who Will Be Directed To The Shelter And The Possibility Of Increasing This Number Is An Estimate And Is The Result Of The Analysis Carried Out By The Ordering Party Based On Previous Years. The Actual Number Of People Who Will Be Directed To The Homeless Shelter May Differ From The Numbers Indicated Above. The Ordering Party Reserves The Right Not To Exhaust The Entire Scope Of The Subject Of The Order And To Increase It Depending On The Needs Of The Ordering Party And The Capabilities Of The Contractor.4. The Ordering Party Allows The Possibility Of Adding The Regulations Applicable In The Shelter To The Contract Template As An Annex, Provided That Their Provisions Do Not Conflict With The Provisions Of The Contract.5. People Who Are To Be Provided With Shelter Will Be Placed In Shelters After Prior Arrangement, Based On A Personal Referral Issued By The Municipal Social Welfare Center In Starogard Gdański. In Particularly Justified Cases, Services May Be Provided Based On Data Provided By Telephone Or Electronically. In Special Cases, An Authorized Employee Of The Municipal Social Welfare Center In Starogard Gdański May Make A Referral By Phone, And The Documentation Will Be Completed Within 2 Business Days.6. The Shelter Should: 1) Provide Shelter 24 Hours A Day, 2) Keep A Register Of People Using The Facilitys Assistance, Referred By The Municipal Social Welfare Center In Starogard Gdański, 3) Carry Out Social Work, Including Individual Programs For Getting Out Of Homelessness, 4) Provide: • Three Meals A Day, Including One Hot Meal; • Basic Personal Hygiene Products, A Change Of Clothes For Clean Ones And The Possibility Of Washing And Drying Dirty Clothes; • Appropriate Social And Living Conditions And Unrestricted Access To Sanitary Rooms;• Contact With A General Practitioner And Health Care Facilities, Painkillers And Cold Medicines;• Feeling Of Safety On The Premises Of The Facility, Respect For Personal Dignity;• Information On Available Forms Of Social Assistance, And, If Necessary, Support In Organizing It;• Legal And Psychological Assistance;5) Cooperate With Mop
