1. The Subject Of The Order Is To Organize And Conduct The Second Of Two Eight-Day Family (Sociotherapy) Trips In 2026 For The Families Of Mops Zgierz Clients, Participants Of The Family Complete Project Co-Financed By The European Social Fund Plus Under The Regional Program European Funds For Łódź 2021-2027. 2. The Trip Mentioned In Section 1 Should Take Place In The Period From July 1, 2026. Until August 31, 2026, In Poland At A Minimum Distance Of 200 Km From The City Of Zgierz, Excluding Accommodation In Towns With Over 50,000 Inhabitants. Inhabitants. Note: In This Procedure, The Region With A Radius Of Not Less Than 25 Km From Jastrzębia Góra Was Excluded Due To The Fact That In 2025 A Sociotherapeutic Trip Took Place In A Given Town.3. The Ordering Party Anticipates That The Trip Will Be Attended By No Less Than 30 And No More Than 40 (Including Children Aged 3-17) Participants Of The Family Complete Project And Their Families (The Final Number Of Family Trip Participants Will Be Provided To The Contractor By The Ordering Party 5 Days Before The Trip, Including A List Of Participants Including Names, Surnames, Dates Of Birth), For The Purpose Of Registering For Personal Accident Insurance.4. As Part Of A Family (Social Therapy) Trip, The Contractor Should Provide A Team Of 3 Specialists (Educator, Psychologist/Therapist, And Animator), Whose Task Will Be To Support And Observe Free Behavior In The Family And Between Other Participants Of The Trip, Dysfunctions And Behavior That Go Beyond The Established Rules And Norms As Part Of The Activities Planned During The Trip Sessions (6 Teaching Hours A Day For 6 Days, Including: Workshops, Diagnosis And Free Time Animation), And Then Prepare Documentation By The Staff Specialists In The Form Of Diagnoses And Reports.5. A Team Of Specialists May Observe Trip Participants For Diagnosis Purposes, Also During A Maximum Of Two Additional Attractions Included In The Stay Program.6. The Trip As Part Of The Order Will Be Attended By 1 Additional Person Acting As The Trip Organizer On The Contractors Side And 2 Employees Of Mops Zgierz Monitoring The Implementation Of The Contract During The Trip Of The Project Participants.7. The Contractor Will Provide Transport, Including Transport To And From The Place Of Accommodation And For Sightseeing. Transport Should Be Organized In A Way That Allows Safe Passage Of All Persons Participating In The Trip, Is Technically Efficient, Comfortable And Equipped With Functional Air Conditioning. The Contractor Will Deliver The Vehicle 30 Minutes Before The Planned Departure In Zgierz, At The Place Indicated/Agreed With The Ordering Party (Target Meeting Point And Place Of Return From The Departure) In Order To Conduct A Drivers Sobriety Test.8. The Contractor Is Obliged To Provide Participants With The Opportunity To Receive Medical Assistance In Emergency Cases.
