Providing Personal Assistance Services For People With Disabilities As A Form Of Generally Available Support In Performing Everyday Activities And Functioning In Social Life, Addressed To People With Disabilities (Program Participants) With A Disability Certificate: A) Children From 2 Years Of Age To 16 Years Of Age - Including The Indications In Points 7 And 8 In The Disability Certificate - The Need For Permanent Or Long-Term Care Or Assistance From Another Person Due To A Significantly Limited Ability To Independent Existence And The Need For Constant Daily Participation Of The Childs Guardian In The Process Of Treatment, Rehabilitation And Education Of The Child B) To A Significant Degree Or C) To A Moderate Degree Or D) Treated On An Equal Footing To The Persons Listed In Points 1 And 2 In Accordance With Art. 5 And Art. 62 Of The Act Of August 27, 1997 On Vocational And Social Rehabilitation And Employment Of Disabled Persons (Consolidated Text: Journal Of Laws Of 2025, Item 913, As Amended) Provided In The Period From The Date Of Signing The Contract/Awarding The Order, But Not Earlier Than From June 19, 2026 To December 14, 2026, In Accordance With The Ministrys Program Family, Labor And Social Policy Personal Assistant For A Person With A Disability For Local Government Units - 2026 Edition, At The Place Of Residence Of The Program Participant In The Municipality Of Rumia, In A Minimum Amount Of 2,395 Hours And A Maximum Of 4,790 Hours (For About 25 People)
