Sample picture must be attached with the RFQ submission. During the evaluation phase, the city reserves the right to do a site visit in order to view and evaluate a physical sample, in line with the specifications reflected in this document. The technical evaluation team may issue a Notice of Intent to undertake a site visit, 1 week prior to the actual site visit date. Service provider must provide details of the address where the sample will be kept for viewing purposes only. GUIDELINE: Wherever reference is made to any Trademark, Name, Patent, Design, Type, Specific Origin or Producer, such reference shall be deemed to be accompanied by the words "or equivalent". If such samples /technical data sheets are not submitted as required in the bid documents or within any further time stipulated by the CCT in writing, then the bid concerned will be declared non-responsive. The Municipal Bidding Document 6.1 is attached, to be eligible for preference points bidders are required to submit a duly completed, current dated MBD 6.1 document with supporting documents with every RFQ submission, failure to submit will result in zero-point allocation. Please note that the MBD6.1 document is valid a year from date of signature. Failure to comply with the above will result in a zero (0) allocation for preference points.
