The Subject Of The Basic Order Consists Of 2 (Two) Parts: 1) Part 1 - Property And Liability Insurance Of The Ordering Party - In The Scope Of: A) Property Insurance Against All Risks, B) Insurance Of Electronic Equipment Against All Risks, C) Civil Liability Insurance, 3. A Detailed Description Of The Subject Of The Order For Parts 1-2 Is Included In Annex No. 6 - Insurance Program. 6. Requirements Specified By The Ordering Party Regarding The Subject Of The Order, Parts 1 And 2, In The Scope Of The Basic And Optional Procurement: 1) The Ordering Party Requires That The Ordering Party (Policyholder/Insured) Is Not Obliged To Cover The Losses Of The Contractor Operating In The Form Of A Mutual Insurance Company By Paying An Additional Premium, In Accordance With Art. 111 Section 2 Of The Act Of 11 September 2015 On Insurance And Reinsurance Activities, 2) The Contractor Must Have General (Specific) Insurance Conditions, Hereinafter Referred To As The General Terms And Conditions, For All Insurance Specified In The Subject Of The Order, 3) The Ordering Party Requires The Contractor Whose Offer Will Be Selected As The Highest Rated To Indicate The Name And Surname Along With Contact Details Of: A) The Person(S) Designated By The Contractor To Cooperate With The Ordering Party During The Implementation Of The Order In The Scope Administrative Activities Related To Ongoing Service (E.G. Issuing Insurance Documents, Clarifying Payment Of Premiums, Preparing Certificates), B) Person(S) Designated By The Contractor To Cooperate With The Ordering Party During The Implementation Of The Order In The Scope Of Supervision Of The Service Process And Loss Adjustment, And These Persons Should Be Indicated In The Public Procurement Contract.7. Confidential Information Provided To Contractors: 1) The Ordering Party Informs That Annexes No. 6 And 7 To The Swz Contain Confidential Information.
