Tenders Are Invited For Providing Care And Assistant Services As Part Of The Project Support And Integration - Social Services For Residents Of The Radzyń Podlaski Commune

1) The Subject Of The Order Is The Provision Of Care And Assistant Services For People Living In The Radzyń Podlaski Commune. The Contractor(S) Will Perform The Services In The Place Of Residence Of The Person Using The Services.4.1.2 The Detailed Scope Of Services Covered By The Subject Of The Order Is Described In Annex No. 7 To The Swz - Description Of The Subject Of The Order (Opz) - Applies To Parts I-Vi Of The Order.4.1.3 The Task Of The Contractor Of This Order Is To Provide Care And/Or Assistant Services Based On An Individual Support Plan Developed For Each Client (Also On Saturdays, Sundays And Holidays). In Justified Cases, The Hours Of Service May Be Changed After Prior Arrangement With The Ordering Party. 4.1.4The Scope Of Support May Be Subject To Modifications Depending On The Needs Of Participants. The Organization Of The Provision Of Care And/Or Assistant Services Takes Into Account The Subjectivity Of The Entitled Persons, Including Respecting The Right To Respect And Protection Of Dignity, Intimacy And The Sense Of Security And Protection Of Personal Rights - Caregivers And/Or Assistants Will Be Supervised By The Ordering Partys Employees, Who Will Contact The Persons Under Their Care, Check Their Satisfaction And Introduce Any Changes In The Support. 4.1.5 The Support Resulting From The Provisions Of The Project Will Be Consistent With The Area Guidelines, In Particular The Minimum Requirements For The Provision Of Social Services In The Local Community (Efs Plus Guidelines). 4.1.6 The Entity Providing Care And/Or Assistant Services Is Obliged To Ensure An Uninterrupted And Appropriate Service Provision Process 7 Days A Week In Terms Of Quality, By Appropriately Determining With Persons Providing Care/Assistance Services The Hours And The Ordered Size And Scope Of Services. 4.1.7 Service Provision Time Means The Time Of Performing The Ordered Scope Of Services (Care/Assistant Services). The Ordering Party Stipulates That The Time For Providing Services Does Not Include The Time Of Travel Or Arrival At The Place Of Service Provision - 1 Hour Means 60 Minutes; 4.1.8 The Contractor Will Provide Services To A Specific Recipient In Accordance With The Procedures Described Below: 1) Information About The Granting Of Services, Their Date And Amount Will Be Provided By Phone Or E-Mail By The Ordering Partys Project Coordinator Based On The Action Plan - Tripartite Agreement, 2) The Contractor Will Receive A Copy Of The Action Plan - Agreement. Tripartite Agreement From The Ordering Party, 3) In A Situation Where The Termination Of The Provision Of Services Takes Place Earlier Than Indicated In The Tripartite Agreement, The Ordering Party Will Inform The Contractor About It By Phone Or E-Mail.4.1.9 The Action Plan Provided To The Contractor - The Tripartite Agreement Will Contain Information Necessary For The Provision Of Services, Such As: 1) Name And Surname And Address Of The Person Qualified To Provide The Services, 2) Type Of Serv
