Description Of The Subject Of The Orderthe Subject Of The Order Is The Service Of Collection And Management Of Municipal Waste (Mixed And Segregated) From Łuw Buildings In Łódź Located At: Piotrkowska 104, Piotrkowska 103, Żeromskiego 87, Gdańska 73, Skrzydlata 50, Łąkowa 40, Ferdynanda Ossendowskiego 1, Piotrkowska 258/260 And Pomorska 21. The Description Of The Subject Of The Order Is Included In Appendix No. 1 To The Swz. The Contractor Is Obliged To Complete The Order On The Terms And Conditions Described In The Draft Provisions Of The Contract Constituting Annex No. 5 To The Swz. Pursuant To The Content Of Art. 95 Section 1 Of The Public Procurement Act, The Ordering Party Requires The Contractor Or Subcontractor To Employ, On The Basis Of An Employment Relationship, Persons Performing Activities Directly Related To The Performance Of The Subject Of The Contract, I.E. Employees Collecting Municipal Waste, Drivers, Employees Performing Administrative And Office Activities, If The Performance Of These Activities Involves Performing Work In The Manner Specified In Art. 22 §1 Of The Act Of June 26, 1974 - Labor Code (Journal Of Laws Of 2025, Item 277, As Amended) Method Of Verifying The Employment Of The Persons Indicated In Section 5 And The Ordering Partys Rights To Control The Contractors Compliance With The Requirements Related To The Employment Of These Persons And The Sanctions For Failure To Meet These Requirements Are Specified In The Draft Provisions Of The Contract Constituting Annex No. 5 To The Swz. The Ordering Party Does Not Require An On-Site Inspection Or Verification By The Contractor Of The Documents Necessary To Complete The Order Available On-Site At The Ordering Party. The Contractor May Only, At His Own Discretion, Conduct An On-Site Inspection.
