The Subject Of The Order Includes Successive Deliveries Of The Active Substance Emicizumab Together With Administration Kits In The Years 2026-2028 In Order To Secure The Continuity Of Treatment Of Patients With Factor Viii Deficiency - Hereinafter Referred To As The Medicinal Product, As Part Of The Implementation Of The Drug Program Of The National Health Fund Entitled Prevention Of Bleeding In Children With Hemophilia A And B - Hereinafter Referred To As The National Health Fund Program, As Well As Organizing Deliveries To Patients Homes, Which Is Considered The Patients Place Of Residence Or Permanent Residence, For The Indicated Number Of Patients, Providing Containers For Collecting Used Disposable Equipment, Collecting And Disposing Of Disposable Equipment And Monitoring The Consumption Of Medicines, As Well As Education On The Proper Implementation Of The Above. Drug Program, On The Terms Specified In The Swz Content. 2. The Offered Subject Of The Order Must Have A Valid Decision, I.E. Be Covered By The Refund As Of May 1, 2026. 3. Total Number (Base Purchase Volume For 2026-2028): 961,368 Mg. 4. Expiration Date Of The Medicinal Product And Composition Of The Administration Kit - In Accordance With Swz. 5. Delivery Volumes And Packaging Proportions Of Administration Sets: To Be Agreed With The Hemophilia Treatment Center. 6. Pursuant To Art. 256 Of The Public Procurement Law. The Contracting Authority May Cancel The Contract Award Procedure Well Before The Deadline For Submitting Offers If Circumstances Occur That Make Further Conduct Of The Procedure Unjustified. 7. Pursuant To Art. 257 Of The Public Procurement Law. The Contracting Authority May Invalidate The Contract Award Procedure If The Public Funds That The Contracting Authority Intended To Use To Finance All Or Part Of The Contract Were Not Awarded To It. 8. The Order Must Be Completed In Accordance With The Requirements Specified In The Swz And In The Draft Provisions Of The Agreement.
