1. The Subject Of The Order Is The Organization Of A Holiday Trip To The Seaside For Adult Pupils Of Family Foster Care Participants Of The Project Entitled: Family - A House Built With Love Iii Implemented By The Municipal Social Welfare Center In Dąbrowa Górnicza Under The European Funds For Silesia 2021-2027 Program, Priority Fesl.07.00 European Funds For Society, Action 07.07 Support For Family, Children And Youth And Deinstitutionalization Of Foster Care.2. Date And Duration Of The Trip: 5-Day Trip (4 Nights) In The Period From July 13, 2026 To August 31, 2026. The Exact Departure Date Will Be Agreed With The Ordering Party After Signing The Contract.3. Place Of Accommodation: Hotel/Guesthouse/Resort Located In A Tourist Town No Further Than 2 Km (2,000 M) From The Baltic Sea.4. Number Of Participants: 10 People (6 Women And 4 Men).5. Transport: The Service Will Consist In Transporting Participants From Dąbrowa Górnicza Al. J. Piłsudskiego 2 To The Place Of Rest (Direct Access To The Place Of Accommodation) And Back. The Means Of Transport Should Be Delivered On The Day Of Departure Between 6:00 A.M. And 8:00 A.M. (The Exact Departure Time Will Be Agreed With The Ordering Party After Signing The Contract, But No Later Than 3 Business Days Before The Departure Day), And On The Day Of Return After Breakfast To The Parking Lot Of The Hotel/Guesthouse/Resort. The Contractor Will Provide Means Of Transport With A Driver And Will Cover All Costs Related To The Transport Service, I.E. Highway Tolls And Parking Fees. The Means Of Transport Should Have Current Technical Inspections, Road Traffic Approval And A Current Third Party Liability And Accident Insurance Policy. The Vehicle Should Be Equipped With An Efficient Heating System And Air Conditioning, And The Year Of Production Cannot Be Earlier Than 2016. The Ordering Party Reserves The Right To Order The Vehicle To Be Inspected By The Appropriate Services Before Starting The Journey. If The Inspection Authorities Prove That The Vehicle Is Inoperable, The Contractor Is Obliged To Provide A New, Functional Vehicle Within 1 Hour. The Contractor Is Obliged To Provide A Functional Vehicle Within 1 Hour Also In The Event Of A Technical Failure That Prevents The Transport Of Trip Participants. The Contractor Is Responsible For The Proper Technical Condition And Efficiency Of The Means Of Transport. The Contractor Is Also Obliged To Provide Stops Along The Route In Designated Safe Places With The Possibility Of Using Toilets.6. Accommodation:A) The Contractor Will Provide Accommodation In Rooms For 2 Or 3 People In A Hotel/Guesthouse/Resort Type Facility;B) The Rooms Must Have Full Sanitary Facilities, I.E. A Bathroom Equipped With A Shower/Bathtub, Washbasin, Toilet, Soap, A Set Of Towels (At Least 1 Large, 1 Small For Each Participant) And Toilet Paper. All Rooms Must Be Equipped With An Appropriate Number Of Single Beds With A Set Of Bedding, A Table, Chairs, Cabinets/Wardrobe For Clothes And A Tv. C
