The Subject Of The Order Is The Supply Of The Active Substance Mirvetuximabum Soravtansinum Used In The B.50 Drug Program For The Treatment Of Ovarian Cancer Patients For The Department Of Surgical Gynecology And Gynecological Oncology For Adults And Girls Usk-2 Pum In Szczecin. The Description Of The Subject Of The Order And The Estimated Quantities Of The Products Were Included In The Unit Price Form (Appendix 2 To Swz, Hereinafter Also Referred To As Fcj). 2. The Ordering Party Requires The Offer Of Medicinal Products In Accordance With The Act Of September 6, 2001, Pharmaceutical Law (Journal Of Laws 2001.126.1381, As Amended), Placed On The Market And Used In The Territory Of The Republic Of Poland In Accordance With Applicable Regulations. 3. The Ordering Party Requires That The Offered Medicinal Products Be Included In Annex B To The Announcement Of The Minister Of Health On The List Of Reimbursed Drugs (Drugs Available In Drug Programs) And The Offered Price May Not Exceed The Financing Limit For A Given Active Substance. 4. Failure To Meet The Condition Described In Points 2 And 3 Will Result In Rejection Of The Offer. 5. The Ordering Party Requires That The Shelf Life Of All Offered Products Is At Least 12 Months, Counting From The Date Of Delivery To The Ordering Partys Headquarters. 6. The Quantities Of The Goods Constituting The Subject Of The Order Specified In The Annex To The Contract Are Estimated Values ​​Used To Correctly Calculate The Offer Price, Compare Offers And Select The Most Advantageous Offer. The Quantity Of The Ordered Assortment, As Part Of The Execution Of The Contract, May Be Reduced Or Increased Within Individual Assortment Items, Depending On The Actual Needs Of The Ordering Party, Provided That The Value Of The Offer And The Contract Of The Selected Contractor Will Not Be Exceeded. The Ordering Party Has The Right Not To Fulfill The Full Quantity And Range Specified In The Contract. In Such A Situation, The Contractor Will Not Be Entitled To Any Claims. The Ordering Party Has The Right Not To Complete The Full Subject Of The Contract. Patients Are Qualified For Drug Programs Based On Criteria Established By The Ministry Of Health And The National Health Fund. The Ordering Party Has No Real Influence On Them, So It Cannot Declare A Minimum Level Of Order Fulfillment. 7. The Ordering Party Pursuant To Art. 30 Section 2 Of The Public Procurement Act, Awards The Contract In Parts, Each Of Which Is The Subject Of A Separate Procedure. 8. The Order Consists Of 1 Task, The Detailed Description Of Which Is Included In Annex 2 Of The Fcj. The Ordering Party Does Not Allow The Submission Of Partial Offers. In Order To Ensure Comprehensive Execution Of The Order, Deliveries Should Be Made By One Contractor. 9. The Ordering Party Does Not Allow The Submission Of Variant Offers. 10. The Ordering Party Requires The Implementation Of The Subject Of The Order For A Period Of 24 Months, Counted From The Date Of Conclusion Of The Contract.
