1. The Subject Of The Order Is The Transportation Of Disabled Students And Their Care To Classes At Schools And Educational Institutions In The 2026/2027 School Year. 2. The Subject Of The Order Is: Part 1 - Route 1 Route I - Bytom, Ul. Nowa 94 - Bytom, Ul. Podhalańska 26 - Bytom, Ul. Powstańców Warszawskich 38 - Bytom, Ul. Marii Konopnicka 3 - Katowice: School And Kindergarten Complex For Deaf And Hard Of Hearing Children, Ul. Grażyńskiego 17 (Changing The Order Of Arrival Is Allowed) Return Via The Same Route After School. Maximum 54.3 Km/Day. Number Of Students: 5 (As At The Date Of Preparation Of The Description Of The Subject Of The Order) + Max. 6 Additional Students During The School Year + Guardian - Provided By The Contractor Note: Means Of Transport Enabling The Transport Of A Child In A Wheelchair3. The Contractor Is Obliged To Provide At Least One Guardian On A Given Route, Whose Duties Will Include: A) Ensuring The Safety Of Children Entrusted To Him From The Moment Of Entering The Means Of Transport Until Reaching The School/Educational Institution And Back, B) Ensuring Proper Care For Children During Each Journey, C) Bringing The Child Entrusted To His Care To The School/Educational Institution.4. The Contractor Is Obliged To Complete The Order In Accordance With The Description Of The Subject Of The Order, The Draft Provisions Of The Contract And The Appropriate Means Of Transport.5. The Subject Of The Order Includes A Service Provided 5 Days A Week, I.E. From Monday To Friday, I.E. A Total Of Up To 188 Days Throughout The Entire Period Of Execution Of The Subject Of The Order.6. In The Event Of Changes To The Days On Which School Classes Will Take Place, There Will Be A Change In The Days On Which Journeys Will Be Made. 7. The Hours Of Transport Of The Child (Enabling Arrival On Time) Will Be Agreed Directly Between The Contractor And The Parent Or Legal Guardian Of The Child.8. It Is Not Allowed To Combine Transport Specified In These Proceedings With Other Transport Tasks Of The Contractor.9. Contractors May Submit Offers For All Lots Of The Contract.10. The Ordering Party Does Not Limit The Number Of Parts Of The Contract That May Be Awarded To One Contractor.11. Detailed Terms And Conditions Of The Contract Are Included In The Draft Provisions Of The Contract - Constituting Annex No. 2 To Swz.12. The Ordering Party Reserves The Obligation For The Contractor To Personally Perform The Service Within The Scope Of The Required License Or Permit To Perform The Profession Of Road Transport Operator - Applies To Each Part.13. The Contractor Is Obliged To Employ, On The Basis Of An Employment Contract, All Persons Performing Work Involving Driving A Vehicle (Driver) And All Persons Performing Work Involving Providing Care During The Transport Of Disabled Persons.
