The Subject Of The Order Is The Collection, Transport And Disposal Of Medical Waste From The Facilities Of The Specialist Hospital. Dr. Alfred Sokołowski At Ul. Sokołowskiego 4 And At Ul. Batorego 4 In Wałbrzych. The Contractor Is Obliged To Collect Waste From Places Of Storage, I.E.: A) From The Waste Warehouse Located In The Hospital Building At Ul. Sokołowskiego 4 - Collection Three Times A Week, I.E. Monday, Wednesday, Friday At From 7.30 A.M. To 1 P.M. B) From The Waste Warehouse Located In The Hospital Building At Ul. Batorego 4 – Collection Twice A Week, I.E. Tuesday And Friday Until 1:00 P.M., Collection Is Allowed Three Times A Week, I.E. Monday, Wednesday, Friday At To 1:00 P.M. The Ordering Party Requires That The Provision Of Services In The Above-Mentioned Scope Was Carried Out In Accordance With The Applicable Legal Provisions In The Republic Of Poland, And In Particular With The Provisions Of: - The Waste Act Of December 14, 2012 (Journal Of Laws Of 2023, Item 1587, As Amended), Including Taking Into Account The Principle Of Proximity Referred To In Art. 20 Of The Act. - Act Of April 27, 2001, Environmental Protection Law (Journal Of Laws Of 2020, Item 1219) - Regulation Of The Minister Of Health Of October 5, 2017. On The Detailed Method Of Dealing With Medical Waste (Journal Of Laws 2017, Item 1975) - Regulation Of The Minister Of Health Of October 21, 2016 On The Requirements And Methods Of Disposal Of Medical And Veterinary Waste (Journal Of Laws 2016, Item 1819) - Act On The Transport Of Dangerous Goods Of August 19, 2011 (Journal Of Laws 2021, Item 756) - Regulation Of The Minister Of Climate Of January 2, 2020. Regarding The Waste Catalog (Journal Of Laws 2020, Item 10) - Regulation Of The Minister Of The Environment Of January 13, 2014 On The Document Confirming The Disposal Of Infectious Medical Waste Or Infectious Veterinary Waste (Journal Of Laws 2014, Item 107, As Amended) And Other Relevant Provisions Regarding The Implementation Of The Subject Of The Order. Estimated Number Of Kg Of Waste During The Period Of Validity Contract (8 Months) – Approximately 226,000 Kg. The Minimum Guaranteed Quantity Is 60% Of The Estimated Value. Waste Collection And Neutralization Will Take Place Successively Over A Period Of 8 Months. The Contractor Will Set Up A Cold Storage Facility In The Place Indicated By The Ordering Party, I.E. A Mobile Trailer For Storing Waste With A Capacity Of At Least 43 M3 And An Inside Temperature Ranging From -4.0Oc To 10Oc. The Contractor Undertakes To Ensure That The Interior Of The Cold Storage Facility Is Kept Tidy And Will Repair It At Its Own Expense And Disinfect Its Interior Each Time The Container Is Replaced. The Contractor Is Responsible For The Costs Associated With Placing The Container In The Place Indicated By The Ordering Party. The Contractors Responsibilities Include Cleaning The Mobile Trailer From Snow And Ice. The Ordering Party Is Responsible For Connecting The Trailer To The Networ
