Tenders Are Invited For Voluntary Group Life Insurance Service Employees, Their Spouses/Life Partners And Adult Children Of Employees

The Subject Of The Order Is The Provision Of Voluntary Group Life Insurance Services For Employees, Their Spouses/Life Partners And Adult Children Of Employees Of The Institute Of Bioorganic Chemistry Of The Polish Academy Of Sciences, In Accordance With The Requirements Of The Ordering Party, Specified In The Detailed Description Of The Subject Of The Order.2. Pursuant To Art. 95 Section 1 Of The Public Procurement Law, The Ordering Party Requires That The Contractor Or Subcontractor Employ, Under An Employment Contract, At Least 2 Persons Performing Activities During The Execution Of The Order In The Scope Of: - Coordinating All Activities Aimed At The Proper Performance Of The Contract And Contacts With The Ordering Party In All Matters Related To The Handling Of The Concluded Insurance Contract. 3. On The Day Of Signing The Contract At The Latest, The Contractor Or Subcontractor Will Provide A List Of Names Of Persons Performing The Activities Referred To In Section 2 Above. Changes In The Persons Referred To Above Will Not Require The Conclusion Of An Annex To The Contract. The Contractor Or Subcontractor Is Obliged To Immediately Inform The Ordering Party About Any Changes In The Above-Mentioned. List. In The Event Of A Change Of Persons Performing The Activities Referred To In Section 2 The Contractor Or Subcontractor Is Obliged To Provide A New List Of Names Of Persons Performing These Activities.4. In Order To Verify The Employment, By The Contractor Or Subcontractor, Under An Employment Contract, Of Persons Performing The Activities Indicated By The Ordering Party In The Scope Of The Execution Of The Order, The Ordering Party May Request In Particular: - A Declaration Of The Employed Employee As To The Nature Of The Contract Constituting The Basis For His Or Her Employment, - A Declaration Of The Contractor Or Subcontractor On The Employment Of An Employee Under An Employment Contract, - A Certified Copy Of The Employment Contract Of The Employed Employee, - Other Documents Containing Information Necessary To Verify Employment Under An Employment Contract, In Particular Name And Surname Of The Employed Employee, Date Of Conclusion Of The Employment Contract, Type Of Employment Contract And Scope Of The Employees Responsibilities.5. No Later Than On The Date Of Commencement Of The Provision Of Services, The Contractor Or Subcontractor Is Obliged To Provide The Ordering Party With A Declaration From The Contractor And/Or Subcontractor On The Employment Under An Employment Contract Of At Least 2 People Performing The Activities Specified In Section 2 Above. This Declaration Should Include, In Particular: The Exact Description Of The Entity Submitting The Declaration, The Date Of Submission Of The Declaration, An Indication That The Activities Related To The Execution Of The Order Are Performed By Persons Employed Under An Employment Contract, Including The Number Of These People, Their Names And Surnames, The Type Of Employment Contract
