1. The Subject Of The Order Is The Supply Of Medical Equipment, I.E. Cardiovascular Diagnostic Devices, For The Needs Of The District Hospital In Pruszków - As Part Of The National Reconstruction And Immunity Plan: Component D Efficiency, Availability And Quality Of The Health Care System Investment D1.1.1 Development And Modernization Of The Infrastructure Of Highly Specialized Care Centers And Other Entities Entitled Improving Access To High-Quality Cardiological Care In An Independent Public Complex Of Health Care Facilities In Pruszków.2. The Subject Of The Order Is Described In Detail In Annex No. 2 To Swz.3. The Scope Of The Order Includes The Supply Of Medical Equipment. 4. At Each Request Of The Ordering Party, The Contractor Is Obliged To Provide Documents Confirming That The Offered Medical Devices Are Placed On The Market And Used In Poland In Accordance With The Provisions Of The Act Of April 7, 2022 On Medical Devices.5. The Conditions For The Implementation Of The Subject Of The Order Were Also Included In The Draft Provisions Of The Agreement, Constituting Annex No. 4 To The Swz.
