The Subject Of The Order Is: Preparation Of Design And Cost Estimate Documentation For The Renovation And Reconstruction Of Residential Buildings - Mińska 15 And Mińska 15 Annex In The Form Of: Preparation Of Multi-Sector Design And Cost Estimate Documentation For The Construction Project Consisting In General Renovation And Reconstruction Of Multi-Family Residential Buildings Located In Warsaw At Ul. Mińska 15 And Mińska 15 - Outbuilding, On The Property Located On Plot No. 14, District 3-02-02. 2. The Buildings Covered By The Study Are Entered In The Register Of Monuments And Included In The Municipal Register Of Monuments Of The Capital City. Warsaw, Including The Front Building At Ul. Mińska 15 Under No. A-1908 And The Outbuilding At Ul. Mińska 15 Under Number A-1934. The Designed Solutions Must Take Into Account The Requirements For The Protection Of Monuments And Be Subject To Arrangements With The Competent Monument Protection Authority. 3. The Scope Of The Order Includes The Preparation Of All Studies, Analyses, Projects And Documents Necessary To Prepare The Investment, Including Obtaining The Required Arrangements, Opinions And Administrative Decisions. 4. The Documentation Must Be Complete From The Point Of View Of The Purpose It Is To Serve, I.E. Enable The Conduct Of The Public Procurement Procedure For Construction Works, Implementation Of Construction Works And Achievement Of The Expected Technical, Energy, Functional And Aesthetic Effect, Including Enabling The Contractor To Prepare A Complete Description Of The Subject Of The Construction Works Contract In Accordance With The Public Procurement Law. 5. The Detailed Scope Of The Order For Stages I-Iii Is Specified In The Contract Template Together With A Description Of The Subject Of The Order And Its Annexes, Which Constitutes Annex No. 4 To The Swz. 6. The Scope Of The Order Also Includes Stage Iv: Providing Authors Supervision And Supporting The Ordering Party At The Stage Of The Contract Award Procedure, As A Result Of Which The Contractor Of Construction Works Will Be Selected (Including, Among Others, Preparing Explanations Of The Content Of The Design Documentation). This Agreement Will Constitute 10% Of The Gross Value Of The Offered Lump-Sum Gross Price Indicated In The Offer Form And Will Be Concluded No Later Than Within 3 Days From The Date Of Initiation Of The Proceedings To Select The Contractor Of Construction Works In Accordance With The Provisions Of The Template Agreement For Authors Supervision, Which Constitutes Annex No. 5 To The Swz.
