By Referring To The Item Number In The Table Included In Annex No. 1.1 To The Swz. 2) With Respect To The Offered Equivalent Products (Substitutes), The Ordering Party Requires That The Offer Be Accompanied By A Declaration That The Offered Equivalent Consumables Are Authorized For Trading In The Territory Of Poland - In Accordance With The Content Indicated In Annex No. 1.2 To The Swz. 3) In The Scope Of The Offered Original Products, The Ordering Party Requires That The Offer Be Accompanied By A Declaration That The Offered Consumables Have The Appropriate Region Code - In Accordance With The Content Indicated In Annex. No. 1.3 To Swz. 4. The Form Of The Evidence In Question, Chapter Vi Point 2 Of The Swz. 5. Complementing And Clarifying The Content Of The Evidence In Question: 5.1. If The Contractor Does Not Submit The Evidence Or The Evidence Submitted Is Incomplete, Order. Will Call For Their Submission Or Completion Within The Specified Deadline. 5.2. Order May Request Explanations From Contractors Regarding The Content Of The Evidence In Question. 6. Duration Of The Contract: The Contract Will Be Concluded For A Period Of 12 Months From The Date Of Its Conclusion, With The Proviso That If The Financial Resources Intended For Its Implementation, Equal To The Gross Price Of The Selected Offer, Are Exhausted Before The Expiry Of This Period, The Contract Will Be Automatically Terminated On The Day Of Exhaustion Of These Funds, With The Exception Of The Provisions Regarding The Guarantee, Which Will Also Be In Force During The Period Of The Guarantee Granted By The Contractor. 7.
