1. The Subject Of The Order Is The Performance Of Construction Works As Part Of The Task Entitled: Modernization Of The Infrastructure Of The Hospital In Płońsk. The Scope Of The Task Includes The Extension Of The Existing Building E Of The Independent Public Complex Of Health Care Facilities. Marshal Józef Piłsudski In Płońsk For The Needs Of The Hospital Pharmacy And The Use Of The Existing Hospital Staircase And Elevator After Their Prior Adaptation (Superstructure).2. The Task Is Co-Financed By The Polish Order Government Fund: Strategic Investment Program, Eighth Edition. The Ordering Party Reserves The Right To Invalidate The Contract Award Procedure If The Public Funds That The Ordering Party Intended To Finance Are Not Awarded To It. The Possibility Of Invalidating The Procedure On This Basis Was Provided For In The Contract Notice.3. The Task Will Be Implemented In The Design, Build And Equip Formula, Which Requires The Contractor To Develop Full Design Documentation (Including Obtaining A Building Permit), Perform Construction And Assembly Works And Deliver And Install The Pharmacys Medical Equipment. The Scope Of Work To Be Performed By The Contractor Includes The Extension Of The Existing Building By Constructing A New Floor, Performing Internal Installations And Adapting The Rooms To Sanitary And Pharmaceutical Requirements, Consistent With The Purpose Of The Building Specified In The Functional And Utility Program.4. Building E, Covered By The Superstructure In Question, Is A Two-Story Hospital Building Constructed In Mixed Technology (Prefabricated Columns And Beams, Brick Walls), With A Basement And A Flat Roof. The Building Is Equipped With Complete Technical Infrastructure (Water And Sewage, Electricity, Telecommunications, Medical Gases). The Facility Includes: The Rehabilitation And Rheumatology Department And The Hospital Kitchen. During Recent Renovation Works, The Building Was Connected With The Other Hospital Buildings By An Underground Tunnel.5. The Scope Of The Order Includes In Particular: 1) Design Works: · Preparation Of A Construction And Installation Inventory Of The Facility To The Extent Necessary To Perform The Subject Of The Order, · Preparation Of A Technical Expert Opinion On The Structure In Terms Of The Possibility Of Superstructure, · Preparation Of Complete Design Documentation Enabling Obtaining A Building Permit And Carrying Out Construction Works On Its Basis, · Obtaining All Necessary Administrative Decisions, Including A Building Permit And Derogations (If Applicable).
