Task No. 1 - Rehabilitation Support Service.2.1. Description Of Task No. 1. Rehabilitation Support Service. Detailed Description Of The Subject Of The Order: 1) Providing A Rehabilitator Service As Part Of Specialized Care Services For Dependent People. 2) The Rehabilitation Service Includes A Wide Range Of Preventive, Educational And Supportive Activities Aimed At Preventing, Slowing Down Or Reversing An Incorrect Lifestyle, Aimed At Improving Motor Functions And The General Health Of Project Participants. The Rehabilitation Service Will Consist In Diagnosing And Treating Diseases And Injuries, As Well As Restoring, Maintaining Or Increasing The Physical Fitness Of Patients. The Scope Of Services Provided Includes: 1) Providing Rehabilitation Support In The Place Of Residence. Based On The First Visit, A Support Plan Will Be Established In Accordance With The Diagnosed Problems, 2) Carrying Out A Diagnostic Procedure, Determining The Problems And Needs Of The Dependent Person In Everyday Functioning, 3) Determining The Forms Of Rehabilitation Interventions Individually For Each Patient, 4) Keeping Documentation Of Home Visits. The Total Number Of Hours Allocated In The Project Is Approximately 432 Hours. Duration Of The Service - At Least 1 Clock Hour (I.E. 60 Minutes). It Is Planned To Provide Support To Approximately 25 People, With The Number Of Participants Being Estimated And May Change During The Execution Of The Order. The Target Group Will Be Qualified In Accordance With The Rules Of The Regulations On Recruitment And Participation In The Project Cus - A New Look At Social Services. 2.1.1 Terms Of Service: A) Form Of Engagement: Mandate Contract, B) Service Completion Date: From The Date Of Signing The Contract To December 31, 2027. 2.1.2. In Addition To Working Directly With Project Participants, The Contractor Will Be Obliged To: A) Perform Its Tasks Diligently, Effectively And On Time. The Contractor Is Obliged To Make Every Effort To Maintain The Participants Motivation To Actively Participate In The Provision Of Services, Using Their Interpersonal Skills In The Field Of Establishing Contacts With Patients, Patience, Divided Attention, Consistency, Sensitivity, Personal Culture And Willingness To Help Others B) Recording The Hours And Completing The Task Related To The Duties Performed On A Monthly Basis, In Accordance With The Applicable Guidelines For Qualifying Expenses, C) The Provision Of The Service Will Be Documented With A Service Implementation Card And The Time Of Providing Services, Signed By Both Parties Each Time The Service Is Provided. At The End Of Each Month, The Contractor Will Immediately Provide The Original Documents To The Ordering Party, No Later Than 5 Business Days. The Contractors Remuneration Will Be Settled In Stages, Monthly, Based On A Vat Invoice Issued On The First Business Day Of The Following Calendar Month, Covering The Hours Of The Rehabilitator/Physiotherapist In A Given Month And After The Orderi
