1) A Detailed Description Of The Subject Of The Order And The Requirements Set By The Ordering Party Are Included In Annexes No. 4-14B To This Swz, Which Constitute A Confidential Part Of The Swz. 2) The Ordering Party Divides The Order Into Two Parts (Tasks). The Ordering Party Does Not Indicate The Maximum Number Of Parts For Which The Contractor May Submit An Offer. Each Contractor May Submit Only One Offer, For One Or More Parts.3) The Subject Of The Order Includes The Comprehensive Insurance Service Of The Bydgoszcz County In The Following Scope: A) Task 1 - Property Insurance (Including The Ferry) And Civil Liability Insurance (Appendix No. 6, 6A, 6B, 6C, 7, 9, 10, 11, 12, 13, 14A To Swz), Including: - Property Insurance From All Risks, - Insurance Of Electronic Equipment Against All Risks, - Insurance Of Construction Machines, - Civil Liability Insurance For Business Activities, - Civil Liability Insurance For The Management Of District Roads, - Ferry Insurance4) Pursuant To Art. 280 Section 3 Of The Public Procurement Law, Appendices No. 4-14B Constitute A Confidential Part Of The Swz And Have Not Been Made Available On The Website Of The Proceedings, Because They Contain Data For The Assessment Of Insurance Risk, Including A Description Of The Subject Of The Contract, Lists Of Property, Registers And Sums Insured.5) The Documents Referred To In Section 4, Will Be Made Available To Contractors Interested In Participating In The Proceedings Upon Their Request. 6) The Application For Access To Annexes 4-14B To The Swz Is Submitted According To The Template (Appendix No. 15 To The Swz) Via The Platform And Should Contain: A) Identification Data Of The Contractor, B) Designation Of The Procedure (Case Reference Number), C) E-Mail Address For Transmitting Documents, 7) The Application Should Be Submitted In Electronic Form And Affixed With A Qualified Electronic Signature, A Trusted Signature Or A Personal Signature By The Authorized Person(S). The Application Must Be Accompanied By An Appropriate Power Of Attorney To Represent The Contractor - If A Person Acts On Behalf Of The Contractor Whose Authorization Does Not Result From The Registration Documents (Krs, Ceidg) Or A Power Of Attorney To Represent Contractors Acting Jointly8) The Application May Be Submitted From The Date Of Publication Of The Contract Notice. It Is Recommended To Submit It In Time To Make The Information Available And To Properly Prepare The Offer.
