Request for proposals for Supply of Ophthalmic Equipment across 14 Target Provinces and 05 Target in Phnom Penh Location:Kandal Province, Kampong Chhnang Province, Kampong Thom Province, Kampong Speu Province, Kampong Cham Province, Stung Treng Province, Takeo Province, Preah Sihanouk Province, Oddar Meanchey Province, Preah Vihear Province, Tboung Khmum Province, Siemreap Province, Ratanak Kiri Province, Svay Rieng Province, Phnom Penh Summary Purpose To support the objectives of the Vision Initiative Project to reach over 870,000 direct beneficiaries, FHF invites legally registered private businesses to submit proposals for the supply of ophthalmic equipment to designated facilities across 14 target provinces and 05 target locations in Phnom Penh. The quantities and technical specifications of the required equipment are detailed in the list below. Please click attachement below for more information. Important Conditions: Delivery Locations: The products shall be safely transported and delivered directly to the following designated hospitals and locations: Kandal Kampong Chhnang Kampong Thom Kampong Speu Kampong Cham Steung Treng Takeo Eye Hospital Preah Sihanouk Oddar Meanchey Preah Vihear Tboung Khmum Siem Reap Ratanakiri Svay Rieng Phnom Penh: Ministry of Health National Program for Eye Health Preah Ang Duong Hospital Khmer-Soviet Friendship Hospital Phnom Penh Municipal Referral Hospital Legal and Registration Requirements Bidders must be legally registered companies or businesses. Copies of valid business registration certificates, patents, and other relevant legal documents must be included in the proposal. Training Equipment Provision Bidders must ensure that all required training equipment is delivered to each designated recipient no later than two (2) weeks after the delivery of the main equipment. International Bidders International bidders must clearly describe their logistics and customs clearance plan, including how they will manage port clearance and shipment of the goods to the specified locations. Scope of Bid and Supporting Documentation Bidders are encouraged to submit proposals that, where possible, cover the complete set of items listed in this RFP. Each submission should include clear images of sample products, and bidders are encouraged to provide website links or online product references to support their proposal.
