Psychologistthe Subject Of The Order Is The Provision Of Specialized Care Services - Psychological Support. The Support Is Addressed To The Target Group Of The Help Closer To People Project, I.E. 20 People Who Need Support In Everyday Functioning, Living In The City And Commune Of Krośniewice, Requiring Support Due To The Inability To Independently Perform At Least One Of The Basic Everyday Activities Due To Their Health Condition. Place Of Order Fulfillment: The Main Place Of Provision Of Services Is The Place Of Residence, Stay Or Study Of The Project Participant, If This Results From Objective Reasons On The Part Of The Project Participant, I.E., Among Others. Communication Exclusion, Permanent Illness, Difficult Family Situation. In Such A Case, The Method Of Providing Support Will Be Consulted Between The Project Participant, The Project Coordinator And The Specialist. The Contractor Covers The Costs Of Traveling To The Place Of Service Provision At His Own Expense. Specialist Care Services Will Be Provided From Monday To Sunday From 7:30 A.M. To 8:00 P.M., With The Exception Of Breaks In The Services Provided Previously Reported By The Ordering Party. The Scope And Duration Of Psychological Services Will Be Determined Based On The Diagnosed Needs Of The Project Participant. The Number Of Hours Of Support Provided To The Project Participant Will Be Determined Individually. A Total Of Max. 750 Hours Of Support. The Number Of Hours Of Psychological Support Depends On The Current Demand, Diagnosed During Work - Max. 750 Hours (Minimum Value = 0). Completion Deadline: 25 Months From The Date Of Conclusion Of The Contract Or Until The Number Of Hours Specified In The Order Is Exhausted. The Contractor Is Obliged To Provide The Service On A Continuous Basis. Rules For The Provision Of Specialized Psychological Care Services: As Part Of The Specialized Care Services Provided, The Contractor Will Be Obliged To: 1) Provide Psychological Support To Project Participants - Individual Meetings, Both In Stationary, Mobile And Remote Form (In The Event Of Objective Circumstances And After Obtaining The Consent Of The Ordering Party). 2) Conduct Individual Meetings Based On Supporting People Who Need Support In Everyday Functioning Due To Their Health Condition. 3) Preparation Of Materials Necessary For The Proper Conduct Of Meetings.4) Providing Psychological Support To Both Adults And Children And Adolescents.5) Maintaining Appropriate Project Documentation, Including: Work Time Sheets.6) Conducting Meetings Based On The Ethical Principles Applicable To Psychologists.7) Cooperation With Employees Of The Municipal And Commune Social Welfare Center In Krośniewice In The Scope Of Keeping Documentation, Supporting Project Participants And Setting The Meeting Schedule.8) Providing The Service Within 10 Business Days From The Date Of Notification Of The Need By The Project Coordinator.9) Covering The Costs Related To Travel To The Place Of Service Provision.10)
