The Subject Of The Order Is The Provision Of Transport Services (Delivery And Pick-Up) Of Dialysis Patients From Their Place Of Residence To The Dialysis Center Of The District Hospital In Chrzanów And Back. Transport Will Be Carried Out By The Contractors Cars In A Group System, Taking Into Account The Patients Place Of Residence, Health Condition And Mobility. The Service Will Be Provided In Accordance With The Schedule (Dialysis Plan) Approved By The Ordering Party. The Contractor Undertakes To Provide Punctual Transport, Ensuring Uninterrupted Operation Of The Dialysis Center.
